perf(server): GitHub activity reads skip PR-wide fields on later review-thread pages (8.9 s → 6.8 s median) - #30294
Reading a GitHub PR's activity re-fetches the whole PR's reviewers, latest reviews, commits, issue-comment and review reactions, and dismissals on every page of review threads. A PR with more than 100 threads pays for those fields again on every later page, and the code throws them away.
Change
listReviewThreadComments in GitHubPullRequestApi.ts sent the full ReviewThreads query for every page, but kept PR-wide fields from page one only.
Now:
The thread selection is a shared ReviewThreadPage fragment.
ReviewThreads reads the first page, as before, and no longer takes $after.
A new ReviewThreadPages query reads later pages and selects only reviewThreads and viewer.
toReviewThreadPage shares its thread and actor mapping with a new toLaterReviewThreadPage. Author avatars and bot logins still accumulate from every page.
The reviewer-refusal retry applies only to the first query, since later pages no longer ask for reviewers.
This is server-only and GitHub-only, and every client benefits. Other providers have their own reads.
Scope and approval
This qualifies for the small, focused bug-fix exception in CONTRIBUTING.md. Work waiting on unrelated work is an obvious performance defect. There is no feature, product default, or contract change.
Verification
Benchmark: a live GitHubPullRequestProvider.getChangeRequestActivity read of #8103 (closed; 155 review threads, 525 conversation entries) with real GitHub credentials. There were 8 timed runs each after a warmup, alternating main and this branch.

Tests in GitHubPullRequestApi.test.ts:
The review-thread cursor test now asserts the second request is ReviewThreadPages. It also asserts that request leaves out reviewRequests, commits, reviewDismissals and latestReviews, and that page one's reviewers are kept.
The reviewer-refusal test covers the first page only, since later pages can't be refused that way.
vp test run on the GitHub API, JSON and provider tests: 196 passed. vp run check:changed passes, including GraphQL codegen.
Not checked: GitHub Enterprise, and an open PR changing between pages. Both before and after already read PR-wide fields from page one only.
The GitHub pull-request API now uses a dedicated query for the initial review-thread page and a threads-only query for later pages. Shared mapping handles thread entries, cursors, and comment actors. Tests cover pagination and reviewer fallback.
The shared page fragment defines thread connection fields. ReviewThreads fetches the initial page, and ReviewThreadPages fetches later pages by cursor.
Map initial and later pages packages/source-control-github/src/server/gitHubPullRequestJson.ts
Shared mapping extracts thread entries, cursors, and comment authors. The later-page mapper returns thread entries and actor data.
Fetch and accumulate review-thread pages packages/source-control-github/src/server/GitHubPullRequestApi.ts, packages/source-control-github/src/server/GitHubPullRequestApi.test.ts
The API reads the initial page and then requests later pages by cursor, up to the existing page cap. Tests cover reviewer fallback, first-page reviewer retention, and later-page bot actor data.

Assert actor metadata from a later page.
A regression that drops later.avatarsByLogin or later.botLogins can pass the current pagination test. Add an actor to the second-page thread and assert that the returned activity retains that actor metadata.

Extend the pagination test around `threadComments` so the second-page thread has
an author with avatar metadata, then assert that `listReviewThreadComments`
preserves that avatar in `conversation.avatarsByLogin`. Keep the existing
first-page reviewer and pagination assertions unchanged.

Addressed the nitpick in 1ecc8e8: the pagination test's second page now has a bot author, and the test asserts its avatar and bot login survive. Removing the later-page avatarsByLogin/botLogins merge makes the test fail.
merged github/main (d694c44d07) with a merge commit, no rebase. two conflicts, both from main's new cached reviewer-refusal helper:
listReviewThreadComments: the first page now reads through main's readRequestedReviewers (refusal cache) instead of the pr's inline retry. later pages still use ReviewThreadPages, which selects no reviewers.
the refusal test keeps the pr's shape (ReviewThreadPages after "last") and expects main's cached behavior on re-read (requestedReviewers: 0). main's "later page refusal" row was dropped since later pages no longer request reviewers.
blacksmith: vp run check:changed passed format, lint, knip, graphql, 11 package typechecks and 172 tests. no other changes. the verification below was done on the pre-merge head 1ecc8e8671.

drove the web client with repo playwright and /usr/bin/google-chrome. opened github pingdotgg/t3code#8103 and its timeline through the running server.
reproduced main's redundant second ReviewThreads query. the pr used ReviewThreadPages for the remaining 55 threads and omitted reviewRequests, commits, reviewDismissals, and latestReviews. observed actual github requests without mocked responses.
the full activity responses were byte-identical: 155 review threads, 525 conversation entries, 100 commits, 3 reviewers, and neither truncation flag set. later-page human and bot avatars, bot flags, reactions, and resolved/outdated state were preserved. sha-256: d0bdd88b39f2b91d1a3108b948a00a5d4e376c32bec8954400e4680359692074.

Unverified finding
Verification ran but could not confirm this finding. It is shown for review, not as a verified issue.
Stop when GitHub repeats a thread cursor.
If later.nextCursor repeats a cursor, this loop requests an earlier page again until REVIEW_THREAD_PAGES is reached. It then returns duplicate threads and an inflated commentCount. Track seen cursors and stop before adding a repeated page. Keep the cursor non-null so the result reports truncation. Based on learnings, pagination must detect an unchanged end cursor instead of continuing.
