perf(server): PR watches wake 14× sooner and stop waiting on unrelated reads (935 ms → 65 ms median) - #30288
A watched pull request could wake its agent close to a second late, or later, for reasons that had nothing to do with that pull request. Each sweep asked every watched PR's host for a fingerprint and read no PR at all until the slowest host answered. A PR with several long review threads then paged the threads one after another. Separately, any PR cache invalidation (after every turn, every watch read, every PR action) waited for all in-flight PR reads in every project. Reading a PR's activity on GitHub also re-fetched the whole PR's reviewers, commits, reactions and dismissals on every page of review threads.
Change
Watch sweep (PullRequestWatchReactor.sweep): every fingerprint is still requested at once, so GitHub's batching is unchanged. Each group's read now starts as soon as its own fingerprint arrives. A semaphore keeps four group reads in flight, as before.
Review-thread tails (readRemarks): separate threads page four at a time. Each thread's cursor chain stays serial. Results are merged in thread order, and the remark watermark still advances only after every tail is complete. A failed page, repeated cursor or missing comment still returns null.
PR read cache (PullRequestReadCache): invalidation took all 512 read permits. Scopes now map to 64 lock stripes. An invalidation waits only for reads sharing its stripe, and a separate one-permit lock serializes revision writes. A read takes its stripes in sorted order, and the revision writer never waits on a lock while holding one, so this cannot deadlock. A read still stores under the revision it started with, so stale in-flight results are still rejected.
GitHub review threads: later pages use a new ReviewThreadPages query that selects only reviewThreads, through a shared ReviewThreadPage fragment. The current code already kept PR-wide fields from page one only, so nothing it used changes. Author avatars and bot logins still accumulate from every page. The reviewer-refusal retry only applies to the first query, since later pages no longer ask for reviewers.
This is server-only, and every client benefits. The GraphQL change is GitHub-only. The reactor and cache changes apply to every provider. Not done: no blanket cap on background GitHub requests. An experiment showed it sped up interactive reads but slowed watch sweeps by about the same amount. No Forgejo reaction fan-out change either (100 comments cost 105 requests), because it needs a separate watch-only read and has no latency measurement.
Scope and approval
This qualifies for the small, focused bug-fix exception in CONTRIBUTING.md. Watch wakes and PR reads waiting on unrelated work is an obvious performance defect. There is no feature, product default, or contract change.

The watch rows inject host latency into the real reactor. Fingerprint, detail, activity and tail call counts are identical on both sides.
The activity read makes the same 7 GraphQL requests on both sides, and all 16 outputs hash identically.
The cache row times invalidate("project:target") while an unrelated read for project:other holds its permits.
Tests:
PullRequestReadCache.test.ts › clears in-flight reads before a new service can reuse them: asserts that another PR's invalidation completes while this PR's waits. It hangs on main.
GitHubPullRequestApi.test.ts: the cursor test asserts later pages use ReviewThreadPages, leave out reviewers, commits and dismissals, and keep page one's reviewers. The reviewer-refusal test now covers the first page only.
vp run check:changed: format, lint, knip, GraphQL, typecheck and 292 tests pass.
Not checked: live end-to-end agent wake latency, a real slow GitHub Enterprise host, and how often a stripe collision makes an invalidation wait (1 in 64 per scope pair). No browser pass, because nothing changes in the UI.

Macroscope's review found this PR approvable — PR watches can process healthy responses sooner, cache invalidation stops waiting on unrelated reads, and later GitHub pages fetch less data. Existing concurrency limits and completeness safeguards keep the performance change bounded without altering product defaults.

Walkthrough
The changes add separate GraphQL operations for initial and subsequent review-thread pages, update page mapping and API reads, and adjust concurrency in pull-request watch sweeps and cache invalidation.
The GraphQL contract separates the initial 100 review threads from cursor-based later pages. Both operations use the shared thread-page shape.
Fetch and map review-thread pages packages/source-control-github/src/server/GitHubPullRequestApi.ts, packages/source-control-github/src/server/gitHubPullRequestJson.ts, packages/source-control-github/src/server/GitHubPullRequestApi.test.ts
The API reads pull-request metadata on the first page, then appends later thread pages and merges their actor data. Tests cover reviewer-field fallback and cursor use.
Coordinate scoped cache reads and invalidation apps/server/src/pullRequest/PullRequestReadCache.ts, apps/server/src/pullRequest/PullRequestReadCache.test.ts
Reads acquire stripes for their scopes. Invalidation acquires all permits for its scope’s stripe, and revision writes are serialized.
The sweep limits active group reads to four and fetches separate review-thread tails concurrently while preserving serial cursor reads within each thread.

A malformed GitHub pagination response can duplicate review threads, but the loop is bounded. This is mergeable with a follow-up cursor guard.

Stop the thread walk when GitHub repeats a cursor.
The later-page loop is bounded by REVIEW_THREAD_PAGES. It does not check whether later.nextCursor was already requested. If GitHub returns hasNextPage=true with an unchanged endCursor, the loop fetches the same page up to nine more times. Each fetch appends the same threads to entries, so the result contains duplicate threads. The dismissals loop below already tracks a seen set. The thread loop needs the same guard.
Stopping on a repeated cursor leaves cursor !== null. The result then still reports reviewThreadsTruncated: true.
Proposed fix
let cursor = first.nextCursor;
let page = 1;
+ const seenCursors = new Set<string>();
// Later pages carry the threads alone; the PR-wide fields above come from the first.
- while (cursor !== null && page < REVIEW_THREAD_PAGES) {+ while (cursor !== null && page < REVIEW_THREAD_PAGES && !seenCursors.has(cursor)) {+ seenCursors.add(cursor);
const later: Pick<
Based on learnings: "detect and reject responses where the server indicates more pages (hasNextPage=true) but the returned end cursor is unchanged from the previous request."
