Reported from a downstream V2 fork.

Problem

A Stop that times out marks the OpenCode session unsettled, so the stopped run's late end is skipped and doesn't finish the next turn. If the event stream drops before that end arrives, the reconnect finds the session idle but leaves the mark in place. When a background subagent then finishes, OpenCode starts a parent execution on its own. That execution's end is the one skipped as the old run's debris, so its continuation turn never ends and the thread keeps showing work. A user prompt clears the mark when it checks session.active. Continuations don't do that check (runWake comes first), so only wakes are affected.

Reproduction

New adapter replay test: ends a background reply's continuation after a lost stream took a Stop's end, in OpenCode2AdapterV2.test.ts. It uses the existing 2.0.18 frame shapes.

  1. A turn starts a background subagent.
  2. The restart interrupt hangs past its timeout, and the turn ends interrupted.
  3. The next turn finds the parent still stopping and fails.
  4. The stream drops, and the reconnect finds only the child running.
  5. The child's report wakes the parent, and the continuation turn takes that execution.

On main the terminals are [interrupted, failed]: the continuation never ends. With the fix they are [interrupted, failed, completed].

The test gates two events with the shared ProviderReplayGate: the stream drop waits until the failed turn has checked the server, and the reply waits until the continuation took its wake. openCode2ReplayRuntime passes a gate through for this.

Fix

A session runs one execution at a time, and each one opens with session.execution.started on the ordered stream. Seeing a start therefore means the run the mark waits on is over: if its end hasn't arrived by then, it was lost with the dropped stream. handleEvent now clears unsettled on any execution start for a thread session. This happens before the event is held for a wake or routed, so the new execution's own end finishes its turn.

A reconnect never replays an old started. The adapter resubscribes with a bare GET /api/event: no cursor or Last-Event-ID. 2.0.18's protocol describes that endpoint as "Volatile by contract: … events during disconnection are missed" (@opencode/protocol groups/event.js). A live 2.0.18 probe agreed: a second subscription opened mid-execution received server.connected, then only the run's later events through session.execution.succeeded, and no session.execution.started.

A prompt whose request failed may start its run only after its turn has failed. That start is the run's own, so it mustn't clear the mark. The mark therefore records whether its run had already started when it was marked (unsettledStarted, from busy):

  • A run that hadn't started only records its own late start, and the mark stays.
  • Only a start after that clears the mark.
  • A reconnect that finds the session idle counts the marked run as started, because by then it's over. A session still active may not have started that run yet, so its next start may still be its own; the run's end clears the mark. reconcile runs whenever a mark is pending.

A prompt rejected without a clear error marks the session too, even when OpenCode never took it. A background reply that starts after that would look like the failed prompt's late start, and its end would be skipped, so its continuation never ended. The continuation turn therefore clears the mark when it takes a reply that is still running: the run the mark waits on is that reply, or was over before it started. A reply that already ended replays without the skip, since its end is its own.

A genuinely late end still gets skipped. If the old run's end arrives before any new start, the mark is still set and consumes it, as before. The existing never lets a timed-out Stop's late end finish the next turn test covers this and still passes.

Verification

  • still checks the server when a failed prompt's execution starts late: a 502 prompt whose execution starts late still makes the next turn check session.active. It fails on this PR's first commit and passes now.
  • still checks the server when a failed prompt starts after a reconnect: the same, with a reconnect that finds the session active before the late start. It fails without the last commit (expected session.active, actual session.prompt) and passes now.
  • ends a background reply's continuation after a prompt failed unclearly and its finished variant (the reply ends before its continuation takes it) time out without the last commit, and each needs its half of that fix. They pass now.
  • ends a background reply's continuation after a lost stream took a Stop's end fails on main's adapter (actual: [ 'interrupted', 'failed' ]) and passes with the fix.
  • vp test run inside unshare -U --map-current-user -p -f --mount-proc:
    • OpenCode2AdapterV2.test.ts + ProviderContinuationService.test.ts: 106/106 (adapter file 88/88).
    • testkit/OrchestratorReplayFixtures.integration.test.ts -t opencode2: 16/16.
  • A live OpenCode 2.0.18 probe of resubscribing mid-execution (above).
  • vp exec tsc --noEmit -p . in apps/server: exit 0.
  • vp lint and vp fmt --check on the touched files: clean.
  • Not run: the full scenario live. A dropped stream during a timed-out Stop can't be produced on demand.

Approvability

Verdict: Approved at 3e33a21

Macroscope's review found this PR approvable — This is a focused OpenCode 2 adapter bug fix that corrects continuation completion after late execution starts and reconnects. Production changes are confined to existing execution-state handling, with targeted tests covering reconnect, late-start, and background-reply scenarios.

…op's end

A Stop that timed out marks the session so its late end is skipped. When the
event stream dropped before that end arrived, the reconnect found the session
idle but kept the mark, so the end of the next execution OpenCode started on
its own (a background subagent's reply) was skipped instead and its
continuation turn never ended. A session runs one execution at a time and each
opens with `started` on the ordered stream, so a start now clears the mark.

…ts late

A prompt whose request failed may still have reached OpenCode, so the next turn
checks `session.active` first. The previous commit cleared that mark on any
execution start, including the late start of that same run, so the next turn
skipped the check and prompted into the run still going.

The mark now records whether its run had started when it was marked. A run not
yet started clears nothing with its own start; only a start after it (a new
execution) clears the mark. A reconnect counts the marked run as started, since
by then it is running or over and its start may have been lost in the gap.

…ter a reconnect

A reconnect counted every marked run as started, even when the server still
ran the session. A failed prompt whose execution had not started yet then
had its own start taken as a later execution's, which cleared the mark, so
the next turn skipped the `session.active` check. A reconnect now counts the
run as started only once the session is idle.

…arly

A prompt rejected without a clear error marks the session `unsettled`. When
OpenCode never took that prompt and a background subagent's report then woke
the parent, the reply's start counted as the failed prompt's late start and
the reply's end was skipped as that run's, so its continuation never ended.

A continuation that takes a still-running reply now clears the mark: the run
the mark waits on is that reply or was over before it started. A reply that
already ended is replayed without the skip, since its end is its own.

Merged in from #14744, these tests started the continuation with a made-up
message id. Wakes are matched by id here, so nothing matched and the
continuation ended at once: the lost-stream test passed even with #14744's
fix removed. They now start the continuation from the offered id and check
that the reply renders.
