fix(codex): restoring a checkpoint works after the Codex session was archived - #29582
If a Codex thread's native session was archived outside T3 Code (for example from the Codex CLI or app sharing the same Codex home), restoring a checkpoint on that thread fails. This happens after a server restart or idle release, when the thread is not loaded in the app-server. The whole restore errors with ProviderAdapterRollbackThreadError: … session <id> is archived. Run codex unarchive <id> to unarchive it first., and files are not restored either. Sending a new message on the same thread works, because normal resume already handles this case (#15389).
Rollback has to resume an unloaded thread before thread/revert, and that thread/resume call had no archived-session fallback.
Change
The existing recovery in resumeThread (match the archived error, call thread/unarchive, retry the identical resume once) now lives in one closure helper, resumeNativeThread. rollbackThread uses it too. resumeThread behaves the same as before.
Scope and approval
Very small, focused fix for an obvious bug, so no prior issue: two Codex adapter paths send the same thread/resume, and only one handles the archived-session error. The fix reuses the recovery from #15389 unchanged.
Overlap search: the local PR index and gh search prs for "codex rollback archived" and "unarchive codex" found nothing. Open Codex adapter PRs #25863, #24538 and #28802 don't touch these lines. #15389 (merged) fixed only the normal resume path.
Verification
Checked against real codex app-server (codex-cli 0.160) in an isolated CODEX_HOME with no model calls. After thread/archive, a fresh app-server returns thread/read → historyMode: "paginated", status: notLoaded, and thread/resume → session … is archived. Run codex unarchive … first. That is exactly the rollback branch.
New test CodexAdapterV2.test.ts › "unarchives an unloaded Codex thread before rolling it back". On main it fails with Caused by: CodexAppServerRequestError: session archived-rollback-thread is archived. With the fix it passes.
Macroscope's review found this PR approvable — Checkpoint restoration now uses the existing unarchive-and-retry behavior when Codex refuses to resume an archived session. Recovery is limited to the selected thread and archive-specific errors, with existing rollback safeguards unchanged.

Walkthrough
The Codex adapter now unarchives a native thread and retries resume when the resume request reports an archived session. A replay test covers this recovery when rollback resumes an unloaded thread.

Summary
Resume and rollback recovery apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts
A shared helper unarchives and retries resume for matching archived-session errors. resumeThread and unloaded-thread rollback use the helper. The replay test checks the resulting native thread ID and conversation head.
Archived unloaded threads can be resumed before rollback. No merge-blocking risk was identified.

This reusable helper performs the resume and unarchive requests without its own operation span. A named span lets traces distinguish archived-session recovery from the enclosing resume or rollback operation. Wrap the helper with Effect.fn("CodexAdapterV2.resumeNativeThread").
As per coding guidelines, “Reusable functions use Effect.fn("Service.operation").” As per path instructions, “Reusable Effect functions should use named Effect.fn spans.”

Verified unchanged head f550fb86c18176db79cfbab554cafbc2c77600da on Linux with Playwright, /usr/bin/google-chrome, codex-cli 0.164.0 and real gpt-6.1-sol turns through the web client and local provider proxy. No mocks or seeded data.
On github/main (30084f0018), created a project and isolated worktree through the app. Two Codex turns wrote version-one then version-two to state.txt. Archived the native session externally, restarted the server, then chose Edit from here → Revert files too on the second prompt. Restore failed with the archived-session error and the file stayed at version-two.
Stopped main and started the exact PR head against the same task-owned state. Retried the same checkpoint. The real protocol trace shows thread/resume failing, thread/unarchive succeeding, the identical resume succeeding, then thread/revert succeeding. The second turn disappeared, its prompt returned to the composer, the native session became unarchived, and state.txt returned to version-one.
Nearby behavior passed: a new provider turn after restore; normal unarchived conversation/file restore; ordinary message resume on a separate archived session, which replied RESUME_OK version-two.
Blacksmith vp run check:changed cbd3e149a130f1d99d2d067c77b9889d64e28755 passed format, lint, knip, GraphQL, typechecks and 135 tests, including all 134 Codex adapter tests. No source changes were needed.
Native iOS/Android and Electron clients, macOS/Windows, remote relay/tunnel connections, other Codex versions and other providers were not exercised on this Linux VM. Idle release was not separately exercised; server restart exercised the unloaded-session path. The recording and uploaded playback/seeking were inspected.

* fix(web): play completion sounds without delay (pingdotgg#29146)
Completion alerts waited two seconds after the client received a completed thread state. Audio was also fetched and decoded only when an alert needed to play, adding a first-play delay.
Notify immediately when the thread shell reports completion, and preload both sounds when a user gesture unlocks audio. Keep completion deduplication and suppression for existing, archived, and stopped runs.
This intentionally removes the continuation grace for completion alerts. A completed run can now notify before an automatic continuation starts. The sidebar's two-second placement grace and server settlement behavior remain in place.
Validation: `vp run check:changed` passes after merging current main, including 40 affected tests, formatting, lint, unused-code checks, affected typechecks, and React compiler checks. Tests cover immediate completion, duplicate suppression across reconnects, preloaded audio reuse, and retrying failed loads on alerts without retrying on every gesture. * fix(desktop): prepare macOS x64 bytecode under Rosetta (pingdotgg#29484)
* fix(server): keep PR watches active after host failures (pingdotgg#29450)
## Problem
PR watches stopped after eight failed host reads, so a prolonged network outage prevented later CI and review updates from reaching the thread.
## Change
Keep watches active after provider failures reported as `failed`. Read detail and activity concurrently without allowing an activity failure to cancel or hide a permanent detail failure. Repeated known permanent failures still stop the watch and notify the agent; rate limits still pause reads. Non-rate-limited GitHub HTTP 403 responses and GraphQL `FORBIDDEN` responses now use a distinct `forbidden` reason so access refusals are counted instead of retried indefinitely. GraphQL error types survive the client mapping, after the existing requested-reviewer fallback.
The provider's `failed` category also includes unclassified host and decode errors. Those continue retrying at the existing two-minute cadence until the watch ends or is stopped. A fixed outage deadline would recreate the recovery failure for longer outages.
## Scope and approval
This is a focused fix for an obvious bug in existing PR watch recovery, covered by the small bug-fix exception in CONTRIBUTING.md. The changes cover watcher recovery, a GitHub failure classification needed to preserve permanent-error handling, and their regression tests. The new server-side `forbidden` reason does not change RPC contracts or add settings.
## Verification
`vp run check:changed` passed, including 154 tests across five files. Regression coverage includes a 30-minute outage followed by one CI-success notification, mixed activity/detail failures, resetting a prior failure count, and repeated forbidden reads stopping the watch. Provider tests distinguish HTTP 403 and exclusively permanent GraphQL refusals from retryable HTTP 503 and mixed or untyped GraphQL errors. The mixed-failure and GraphQL classification cases failed before their fixes. Seven focused requested-reviewer fallback/refusal tests also pass.

Problem
If a Codex thread's native session was archived outside T3 Code (for example from the Codex CLI or app sharing the same Codex home), restoring a checkpoint on that thread fails. This happens after a server restart or idle release, when the thread is not loaded in the app-server. The whole restore errors with
ProviderAdapterRollbackThreadError: … session <id> is archived. Run codex unarchive <id> to unarchive it first., and files are not restored either. Sending a new message on the same thread works, because normal resume already handles this case (#15389).Rollback has to resume an unloaded thread before
thread/revert, and thatthread/resumecall had no archived-session fallback.Change
The existing recovery in
resumeThread(match the archived error, callthread/unarchive, retry the identical resume once) now lives in one closure helper, resumeNativeThread.rollbackThreaduses it too.resumeThreadbehaves the same as before.Scope and approval
Very small, focused fix for an obvious bug, so no prior issue: two Codex adapter paths send the same
thread/resume, and only one handles the archived-session error. The fix reuses the recovery from #15389 unchanged.Overlap search: the local PR index and
gh search prsfor "codex rollback archived" and "unarchive codex" found nothing. Open Codex adapter PRs #25863, #24538 and #28802 don't touch these lines. #15389 (merged) fixed only the normal resume path.Verification
codex app-server(codex-cli 0.160) in an isolatedCODEX_HOMEwith no model calls. Afterthread/archive, a fresh app-server returnsthread/read→historyMode: "paginated",status: notLoaded, andthread/resume→session … is archived. Run codex unarchive … first. That is exactly the rollback branch.CodexAdapterV2.test.ts› "unarchives an unloaded Codex thread before rolling it back". On main it fails withCaused by: CodexAppServerRequestError: session archived-rollback-thread is archived. With the fix it passes.vp test run src/orchestration-v2/Adapters/CodexAdapterV2.test.ts: 134/134 pass, including the existing fix(codex): resume archived native sessions #15389 archived-resume table.vp run check:changed: all pass (format, lint, knip, typecheck, tests).Reviewed with sol-loop: 1 round with GPT-6.1 Sol on high. Sol verdict: approved.
