fix(server): retry Codex capacity failures with empty input - #25611
Codex can end a turn with serverOverloaded without retry advice, causing T3 to fail work that could resume. Retry these root turns up to five times in the existing conversation, with delays from 2 to 30 seconds and empty input so the user message is not resent.
The run stays active while waiting. Stop and steering reach the replacement turn during startup, including when notifications arrive before the RPC response. Native child results can wake a recovered parent, while a final failure or interruption prevents that wake-up.
Validation: vp run check:changed and all 100 Codex adapter tests passed. Protocol replays cover recovery, exhaustion, Stop during waiting and startup, steering, notification ordering, completed turns before RPC replies, late child results, and Stop racing a capacity retry during goal pause. The orchestrator replay keeps one user message, records the failed and recovered native turns, and completes the same run.
- Continue failed overloaded turns from persisted history, with bounded backoff
- Keep retries interruptible and preserve one user message across continuations
Macroscope's review found this PR requires human approval — This PR adds automatic Codex capacity recovery across production turn lifecycle, lineage, child-result, Stop, steering, and goal-pause paths, with substantial asynchronous state management. The scope is broader than a self-contained bug fix, and an unresolved high-severity retry-lineage risk remains flagged.

Walkthrough
CodexAdapterV2 retries eligible root turns that fail with serverOverloaded. It tracks retry state across continued turns and updates finalization, Stop, and steering behavior for pending or completed retries.

Stop targets the latest continued turn and handles pending retries. Steering waits for a pending retry to provide its active context. Tests cover Stop during retry and steering during continuation.

A capacity retry creates a new ActiveCodexTurnContext, but an already-running child keeps the earlier parent context until that child starts another turn. Checking only input.context can miss the live child.
A failed retry can therefore emit turn.terminal without childTurnsContinue: true. RunExecutionService then cascades and clears open child projections. An interrupted retry can publish its terminal before the child settles.
Check every context in the continued run in emitOrDeferRootTerminal and flushReadyRootTerminals. Route retry start failures through the same descendant-aware terminal logic instead of calling emitRootTerminal directly.

Addressed the outside-diff descendant finding. Terminalization and deferred-terminal checks now inspect children of every turn in the continued run. Retry-start failures use the same descendant-aware path. Replays verify that a failed retry leaves running children intact and an interrupted retry waits for them without scheduling another continuation.
kept @Yash-Singh1's bounded backoff, empty-input recovery, and replay coverage. merged current main and simplified startup state. fixed five reproduced interleavings involving stop after startup failure, child wakes during/after startup, persisted steering, and retry state leaking into the next task.
verification: the recovery test fails against current main. all five added regression cases failed before their fixes. blacksmith vp run check:changed passes, including lint, typecheck, knip, and 124 tests in three existing files. no new test files.
