Problem

On Linux, libuv's spawn() returns only after the forked child has run chdir(cwd) and the execvp PATH search, and the server's event loop is blocked for that whole time. When a project directory, or a PATH directory searched before the binary, is on a network mount that stops answering, every spawn freezes the whole server and all clients show "did not respond" until the mount recovers.

Reproduced on main cefb544af3 on Linux (Node 26, libuv 1.52.1) with a FUSE mount that answers each lookup after 5 s:

  • The reporter's spawn-stall.mjs: cwd on the mount blocked the loop 5,003 ms, the mount first on PATH 10,002 ms, a missing command with the mount last on PATH 10,003 ms.
  • A real dev server (fresh database) with the mount first on its PATH: event-loop-stalls.ndjson logged delayMaxMs: 9846 during the git spawn under ProjectService.listPage → RepositoryIdentityResolver.resolveCacheKey → processRunner.runProcessCore, the same chain as the report's trace.

Change

  • apps/server/src/process/NonBlockingSpawner.ts wraps the server's Effect ChildProcessSpawner on Linux. macOS and Windows get the native spawner unchanged.
  • Wiring in server.ts. The wrapper sits in the platform services. It also overrides the HTTP layer's output, because NodeHttpServer.layer re-exports NodeServices and its native spawner. It is also provided to ProcessRunner.layer where that layer is first built (it is memoized). Without the last two, provider drivers and every ProcessRunner user (git, port scanner) kept the native spawner.
  • The chdir and the real PATH search move into the child. Every non-shell command runs as /usr/bin/env [-C <cwd>] -- <command> <args…>, so the parent only ever execs /usr/bin/env and never passes a cwd to libuv. GNU env leaves the environment untouched (including BASH_FUNC_*%% exports used by environment modules), keeps argv as given, and searches PATH with glibc execvp.
  • Native errors are kept. Before spawning, async checks on the cwd (search permission) and the command (each PATH candidate, or the explicit path) still produce the native NotFound / PermissionDenied errors, which callers use to report a CLI as not installed. PATH candidates are joined without normalization. The checks are queued per directory, and at most two run at once across all directories. Hung mounts therefore hold at most two of libuv's four threadpool threads, which also compress every WebSocket frame, however many spawns hit them. A spawn whose check waits behind two hung checks waits for a free slot. Nothing is cached.
  • Shell mode. A command with shell set (on Linux, ACP client terminals with shell commands) is rewritten the way Node runs it on Unix, <shell> -c <script>, and takes the same trampoline.
  • The cwd is checked as given and entered resolved, as the native spawner does, so missing/.. still fails with the native NotFound. Its errors are built from the capped check's result, with no second filesystem call.
  • sh fallback. One probe when the layer is built decides whether env -C is supported. Where it isn't (busybox), and for command names containing =, a /bin/sh -c 'cd -- "$1" && shift && exec "$@"' trampoline is used instead.
  • Claude. ClaudeAdapterV2's direct /bin/sh spawn now changes directory inside its script instead of passing a native cwd. The capabilities probe in ClaudeProvider.ts uses that same spawn instead of the SDK's own, which handed the workspace to libuv as a native cwd.
  • Muse. On Linux, createMuseSdkHostEffect launches Muse as /usr/bin/env -C <cwd> -- <muse> …, which keeps every environment entry, as the spawner's env path does. A command containing =, or a lone -, which env cannot run, keeps the native launch. The Muse SDK spawned it natively, so a missing muse searched every PATH directory on the event loop: the report's 20.7 s createMuseSdkHostEffect trace.
  • A command named - takes the sh path, since GNU env reads a lone - as -i even after --.
  • Explicit paths through a file or a symlink loop fail as BadResource with their errno, where Node's native spawn throws a defect.

Scope and approval

Fixes #30183

Verification

  • NonBlockingSpawner.test.ts, run against real processes on Linux:
    • Covers cwd, argv, explicit-path and PATH-search behaviour on both trampolines.
    • Compares wrapped against native for a non-executable tool ahead of a working CLI, a link/.. PATH entry, and an unenterable cwd.
    • Checks that an exact env without extendEnv reaches the child unchanged and that a BASH_FUNC_x%% export survives.
    • An invariant test checks that the native spawner only ever receives the trampoline and never a cwd.
    • checks at most two paths at once, however many directories hang: three spawns into distinct hung cwds (access mocked to hang) never run more than two checks. Without the cap it fails with expected 3 to be 2.
    • Shell mode: runs a shell-mode command the way the native spawner does, and the trampoline invariant test includes a shell-mode command. That invariant test fails on 4533dd0.
    • fails a missing cwd the way the native spawner does covers missing and missing/.., and fails on bc67d6e for missing/...
    • fails an explicit path through a file or a symlink loop as a bad resource.
  • sdk.test.ts (provider-muse): starts Muse in its cwd and environment without handing libuv the cwd or PATH search fails on 2ffa64f (native cwd) and on e6b78be (dash dropped BASH_FUNC_x%%).
  • The PATH parity test also runs an executable named -; it fails through env on 2ffa64f.
  • ClaudeCapabilitiesProbe.test.ts: starts the probe CLI in its workspace through the shell trampoline fails on 74d2b06, where the probe had no spawn hook.
  • All 16 spawner tests also pass under CI's traced runner (vp run test-cache, which preloads vite-plus's fspy). fspy's execve stops the PATH search at a script whose interpreter is missing, so that row now uses a non-executable file.
  • ClaudeAgentSdkQueryRunner.test.ts: the CLI runs in its cwd, and a missing cwd exits 125.
  • processRunner.test.ts, VcsProcess.test.ts and vp run check:changed (format, lint, knip, typecheck, tests) pass at d0c66c3. The runtime transcripts below were taken at 4533dd0; later commits change shell-mode commands, cwd validation, explicit-path errors, the Claude probe spawn, cd -P in the Claude trampoline, and Muse startup.
  • Server-only change with no render path, so there are no captures. The proof is the runtime transcript below. "before" is NodeServices.layer, the composition on main; "after" is NonBlockingSpawner.layer over it, as server.ts wires it on this head (6df32aa). FUSE mount at 5 s per lookup:

Unrelated threadpool work during 6 spawns into the hung cwd plus 6 distinct missing commands with the hung directory on PATH (6df32aa):

Unrelated threadpool work while 6 spawns run at once into 6 distinct directories on the hung mount (5 s per lookup), sampled every second. "before" is b38ff31, which only serializes checks per directory; "after" is e82895b with the two-check cap. Excerpt: every stalled sample is shown, and the samples left out all took under 3 ms:

Before the cap, the six checks took all four threadpool threads, so a 64 KiB deflate waited 29 s (that is WebSocket compression on the real server). With the cap, unrelated work stays under 3 ms. The hung spawns themselves finish later, at 70 s instead of 55 s, because only two of their checks run at a time.

Through the real server (node apps/server/src/bin.ts --auto-bootstrap-project-from-cwd, fresh --base-dir, scratch git project, T3CODE_DEV_SEED=0). A --require preload logs every child_process spawn: argv and native cwd. "before" is e82895b, which had the wrapper only in the platform services; "after" is 4533dd0:

At 4533dd0 the one native spawn left was the Claude capabilities probe, which went through the SDK's own spawn. At 8ef1942 it goes through the same /bin/sh trampoline as the adapter, and the spawn log shows no native cwd for any child.

Not covered:

  • Spawns that bypass the Effect spawner: serviceLauncher, DocumentText and node-pty terminals.
  • A real NFS server (FUSE stands in for it).
  • busybox and Alpine hosts, which use the spawner's sh fallback. Muse there would need GNU env -C.
  • macOS, where libuv's posix_spawn chdir likely blocks the same way. The Effect spawner wrapper is Linux-only; the Claude shell trampoline also runs on macOS, where it behaves the same. Muse keeps its native launch off Linux.

…r on spawn

On Linux, libuv's spawn blocks the event loop until the forked child has
run chdir(cwd) and the execvp PATH search, so a hung network mount under a
project or a PATH entry froze the whole server.

The server now wraps Node's ChildProcessSpawner on Linux: bare command
names are resolved against the child's PATH with async fs calls (hits
cached 30s, misses never cached, concurrent lookups shared), and commands
with a cwd run through `/bin/sh -c 'cd -- "$1" && shift && exec "$@"'`
so the chdir happens after the parent is released. A missing command
still fails with a NotFound PlatformError, and a missing cwd fails through
the same fs.access check as before.

Fixes #30183

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Explicit paths now fail with NotFound/PermissionDenied as natively, the child
shell does the real PATH search (so a stale script ahead of a working CLI no
longer breaks it), PATH entries and the cwd are never normalized lexically, and
a binary on a hung mount no longer blocks the parent when no cwd is given. The
30-second lookup cache is gone; only in-flight lookups are shared.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…ude off a hung cwd

- Linux commands now run as `/usr/bin/env [-C <cwd>] -- <command> <args...>`,
which passes the environment through untouched (dash dropped BASH_FUNC_*%%
entries and added PWD/OLDPWD). Hosts without `env -C`, and commands
containing `=`, keep the /bin/sh trampoline.
- Parent-side checks run at most one per directory, and checks of one path
share the one in flight, so a hung directory holds one threadpool thread.
- A cwd is resolved lexically and checked for search permission, failing
  PermissionDenied like a native spawn.
- The Claude process-group shell enters the cwd itself instead of Node's
spawn doing chdir in the parent's fork.

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — The change moves process startup filesystem work out of the server’s blocking spawn path to prevent hung mounts from freezing it. The new file-level nodeBuiltinImport:off diagnostic suppression requires human review.

Walkthrough

The server adds a Linux-specific child-process spawner that prepares commands through an environment or shell trampoline. Claude process-group commands now change to the requested directory inside the shell. Tests cover working-directory handling, executable lookup, environment forwarding, concurrency limits, and spawn errors.

Changes: Linux child-process spawning

Layer / File(s) Summary
Prepare Linux commands for spawning
apps/server/src/process/NonBlockingSpawner.ts
On Linux, the spawner checks working directories, searches executable candidates, and prepares commands through /usr/bin/env -C or /bin/sh. It prepares pipeline commands recursively and limits concurrent filesystem checks.
Register and validate the Linux spawner
apps/server/src/server.ts, apps/server/src/process/NonBlockingSpawner.test.ts
The server registers the Linux-specific spawner layer. Tests compare command arguments, working directories, PATH lookup, environment handling, errors, concurrency, and trampoline commands with native spawner behavior.
Select Claude process directory in the shell
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAgentSdkQueryRunner.test.ts
The shell changes to the requested directory before running the watcher. Relative directories receive a ./ prefix. A failed directory change exits with status 125. Tests cover valid and missing directories.

Full details: Out of Scope Changes check

Explanation

Each changed file in the current summary supports [#30183]. NonBlockingSpawner.ts moves cwd changes and PATH search into the child. It also adds the native-error checks and the two-check concurrency cap. server.ts wires the layer into the HTTP server and ProcessRunner.layer. ClaudeAdapterV2.ts moves the directory change into the shell script, so no native cwd is passed. ClaudeAgentSdkQueryRunner.test.ts and NonBlockingSpawner.test.ts verify these behaviors. The summary shows no unrelated change. The 'Not covered' items are gaps in coverage, not out-of-scope changes.

Full details: Description check

Explanation

The description is thorough and follows the template's sections. It has a Problem section with reproduction details, a Change section that explains the fix across components, a Scope and approval section that links the issue (fixes #30183), and a detailed Verification section with runtime transcripts and test results. It also names the models used and lists items not covered. The Scope and approval section is brief: it links the issue but does not include an explicit maintainer approval comment or explain why the change is in scope beyond that link. This is a minor gap. The change is large, so the approval requirement is the main open point.

Full details: Linked Issues check

Explanation

[#30183] requires that a stalled filesystem under one project or PATH entry delay only the affected work, and that the rest of the Linux server keeps responding. The PR covers spawns made through the Effect ChildProcessSpawner. server.ts wires NonBlockingSpawner.layer, and the PR transcript reports event-loop lag of 3 to 33 ms for the covered cases. The issue also names a stall path that remains uncovered. The Muse missing-command lookup (UsageService.resolveTranscriptDirs -> MuseUsageHome.initialize -> createMuseSdkHostEffect) blocked for 20.7 s with muse searched across NFS PATH entries. The PR description lists Muse under 'Not covered', because the Muse SDK calls node:child_process directly and bypasses the Effect spawner. The changed files in the current summary (ClaudeAdapterV2.ts, NonBlockingSpawner.ts, NonBlockingSpawner.test.ts, ClaudeAgentSdkQueryRunner.test.ts, server.ts) include no Muse change. The previous finding for this path still holds at the current head. The issue also lists provider and CLI probes as affected. Those probes can reach the Muse path, so the uncovered path can still block the event loop. Limits: I did not inspect the Muse SDK source. The conclusion rests on the issue text, the PR description, and the changed-file list.

Resolution

Route the Muse SDK host process spawn, and any other provider probe that uses node:child_process directly, through a non-blocking mechanism. Options include the NonBlockingSpawner layer or equivalent cwd and PATH isolation that keeps the event loop free. Add a regression test that stalls a PATH entry on that path and checks event-loop lag, in the same way as the existing spawner tests. If the Muse SDK cannot be routed, state that constraint in the PR and provide a mechanism that isolates its PATH lookup from the server event loop.

I withdraw my inline suggestion to remove the bash test: it exercises the no-cwd trampoline separately from the environment test.

Checks were only serialized per directory, so spawns into four distinct

Preserve the supplied environment in the /bin/sh fallback.

When /usr/bin/env -C is unavailable, this path starts /bin/sh, changes directory, and executes the command. The shell drops names such as BASH_FUNC_x%% and adds PWD or OLDPWD. A Bash child can therefore lose an exported function, and any child with a cwd receives an environment different from the one supplied by the caller. Use a fallback that preserves arbitrary environment names without adding shell-managed variables.

On CodeRabbit's outside-diff note about the /bin/sh fallback (NonBlockingSpawner.ts viaShell): this is known and stays as is. The fallback only runs when the layer's startup probe finds no working /usr/bin/env -C, which in practice means busybox/Alpine. GNU coreutils has had env -C since 8.28 (2017). Any glibc host that can run Node 26 takes the env path, and that path keeps BASH_FUNC_*%% exports and adds no PWD (covered by hands the child exactly the environment it was given). Busybox has no env -C and no other portable way to chdir and exec without a shell. The comment above viaShell documents the trade-off.

Keep shell-enabled commands off the native cwd path.

prepareStandard returns shell-enabled commands unchanged. The Devin ACP terminal path can produce such a command with a project cwd. Effect then forwards that cwd to Node's native spawner. A slow mount can block the server event loop during native spawn and delay unrelated WebSocket and RPC work.

Route shell-source commands through a child-side shell trampoline that performs cd inside the child, and leave native cwd unset. Preserve the existing shell command and shell executable semantics.

ProcessRunner was first built from the CLI's native NodeServices. Git and

On CodeRabbit's outside-diff note about shell-enabled commands keeping a native cwd: fixed in bc67d6e. On Linux the only shell-mode spawn is ACP client terminals with shellCommands (resolveSpawnCommand sets shell only on Windows and for git on macOS). Shell-mode commands are now rewritten the way Node runs them on Unix, <shell> -c <script>, and take the same env -C trampoline. New test: runs a shell-mode command the way the native spawner does. The trampoline invariant test now includes a shell-mode command, and it fails on the previous head.

The cwd was checked after lexical resolution, so missing/.. passed where
the native spawner fails, and a failed check fell back to an uncapped
FileSystem.access. Check the raw cwd through the capped probe and build the
native error from its result.

Validate the raw cwd under the shared limiter on every path.

enter can accept E/missing/.. because it validates the resolved directory and skips fileSystem.access(cwd) when that directory exists. This differs from the native spawner, which validates the raw cwd with F_OK before resolving it. The conditional access also bypasses the two-slot limiter for non-directory results. Always validate the raw cwd under the existing limiter, then retain directory for execution.

The capabilities probe used the Claude SDK's own spawn, which hands the
project directory to libuv as a native cwd.

On CodeRabbit's two outside-diff notes from its review of bc67d6e:

  • Validate the raw cwd under the limiter: already fixed in 74d2b06. enter checks the cwd as given through the capped probe and enters the lexically resolved path, as the native spawner does. It builds the native error from the probe result, with no separate fileSystem.access call. fails a missing cwd the way the native spawner does covers missing/...
  • Physical cd in the Claude trampoline: fixed in 2ffa64f (cd -P). starts the CLI in its cwd… now checks that link/.. lands where the kernel's chdir does; it fails without -P.

…mmand named - runs

Muse SDK startup spawned natively, so a missing muse searched every PATH
directory, hung mounts included, on the event loop: the issue's second trace.
GNU env reads a lone - as -i even after --, so that name takes the sh path.

… child

dash, /bin/sh on Debian and Ubuntu, drops entries such as BASH_FUNC_x%%
before it execs, so launch Muse through GNU env -C on Linux instead.

GNU env reads such an operand as an assignment even after --.

[Bug]: Linux server freezes for minutes when a network filesystem stalls, because spawn() waits for the child's chdir and PATH lookup
