fix(opencode): long command turns no longer fail when the command request times out - #27677
An OpenCode turn whose prompt starts with a known command (/my-skill …) is marked failed with "Provider turn failed." about 5 minutes after it starts, while OpenCode keeps running the command. OpenCode's POST /session/{id}/command does not respond until the whole command run finishes. Under Node, fetch (undici) gives up waiting for response headers after 300 s (the SDK's req.timeout = false only applies on Bun). The request's error handler then finalized a turn that had been admitted long before.
Reproduced on main (6575ce3) with real opencode serve 1.18.18 and a project command /slow 330 that runs sleep 330: the turn failed 300 s after session.command was sent, and OpenCode finished the command 37 s later.
Change
In submitPrompt (packages/provider-opencode/src/server/adapter.ts), the forked session.command request no longer fails the turn once the command's user receipt has admitted it. From then on the event stream (session.idle / session.error) decides the outcome, the same way it already does for promptAsync turns. Failures before admission still fail the turn, and so does the existing 10 s admission timeout. Losing the event stream still fails the turn as before.
New test keeps an admitted native command running when its HTTP request times out in packages/provider-opencode/src/server/adapter.test.ts: on main it fails with expected 'failed' to equal 'completed'; with the fix it passes. The whole file passes 38/38, and vp run check:changed passes (format, lint, knip, typecheck of 10 packages, 39 tests in 2 files).
Runtime transcripts. Both runs used the web dev server on a fresh database, OpenCode 1.18.18 with anthropic/claude-haiku-5-5, Node 26.12, and a project command .opencode/command/slow.md that runs sleep $ARGUMENTS && echo slow-done. The prompt was /slow 330.

Mechanism check: a standalone Node script called session.command through the same SDK client against the same server. The call rejected at 300.8 s with TypeError: fetch failed, cause HeadersTimeoutError (UND_ERR_HEADERS_TIMEOUT). The session was still busy at that moment and finished normally at 335.9 s. A 20 s control command resolved only after 22.8 s, which confirms the endpoint holds the response until the run ends.
.opencode/command/slow.md: Use the bash tool exactly once to run `sleep $ARGUMENTS && echo slow-done` (set the bash tool timeout to 900000 ms). Then reply with exactly: DONE
Not checked: the OpenCode 2 adapter (packages/provider-opencode/src/server/v2/), which has its own command path and transport and is not changed here. Desktop (Electron) runs the same server adapter and was not run separately. No UI changed, so there are no captures.
Model and harness: Claude Opus 5.5 (lead, red test, implementation, review B) and GPT-6.1-Sol (review A), via Claude Code and Codex in T3 Code.
No code changes; evidence and verification added below. The earlier transcripts were taken at 6575ce3 / 157d524. These were taken at the current merge-base and head.
Evidence
Server-only, so there are no images. Rows are timestamped transcripts. M1 uses the real dev app on fresh homes (T3CODE_DEV_SEED=0) driving real opencode serve. M2, M3 and the C rows are labelled adapter-harness recordings (mocked SDK and native events, real adapter). Diff0 keeps every recorded field. DiffT keeps every event and field and normalises only the fields named at its top (UTC/offsets, generated IDs, checkout paths).
Environment: fs-11, Linux 6.1.102 x86_64, Node 26.12.1, OpenCode 1.18.18 (SDK 1.15.13), openai/gpt-6-luna via CLIProxyAPI, prompt /slow 330. Viewport, theme and DPR are N/A.
What else changed (every difference in the diffs):
M1: before, the run records the HTTP error and leaves usage unavailable. After, it records the late bash result, DONE, complete usage, and a final checkpoint record (status missing, so no usable checkpoint is claimed). IDs, ports, paths, timestamps, timeout jitter and model metadata also differ.
C6 keeps OpenCode's own failure message and code. C7 changes from provider_error/reusable to transport_error/broken, because stream loss now ends the turn instead of the HTTP error. C3/C10: interrupt succeeds, where before it reported an inactive turn.
No change outside the turn outcome of admitted commands. C2, C5, C8 and C9 (optional controls) were not captured. C3, C6 and C7 cover only the variants listed.
Evidence round (evidence-only, no code change): proof audit pass 1 derived the matrix (minimum M1-B, M1-A, M2, M3). Pass 2 found the normalised diffs too lossy and one mislabelled C4 caption; both were regenerated from the same recordings. The final pass 2 returned SUFFICIENT. The auditor replayed the M2 harness independently (18/18 records identical apart from timestamps and IDs) and regenerated M1-DiffT, M2/M3 DiffT and C4 with a 0-byte delta.
Tests and reviews from the original round are unchanged: adapter.test.ts 38/38. The new regression test fails on main.
CI on 134aea7: Build and Typecheck, Check and the other required suites pass. Preview jobs are skipped.
Unverified: the OpenCode 2 adapter (separate path, not changed). Desktop runs the same server adapter.
Model and harness: Claude Opus 5.5 (lead) and GPT-6.1-Sol (proof audit, capture), via Claude Code and Codex in T3 Code.
…27664)
These tests reproduce #27664: once a native OpenCode command is admitted
by its user receipt, a later session.command request failure (Node's
undici headers timeout after 300 s) still finalizes the turn as failed
instead of letting session.idle complete it.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…uest times out
OpenCode's POST /session/{id}/command only responds once the command run
finishes, so Node's fetch drops it after its 300 s headers timeout while
OpenCode keeps working. The request's failure handler then failed the turn
even though the command had already been admitted by its user receipt.
Once the receipt has admitted the command, a failed command request no
longer finalizes the turn; session.idle / session.error decide the outcome,
as they do for promptAsync turns. Failures before admission still fail it.

Macroscope's review found this PR approvable — Admitted OpenCode commands now rely on provider events rather than a late HTTP timeout to determine completion. Admission timeouts and event-stream failure handling remain intact, keeping the change narrowly scoped.

Handing off #27664. All my verification ran on physical corex, Linux x86_64. I did not run a desktop/mobile client or a real OpenCode CLI/model turn, and I have not tested this PR.
Reproduced the transport failure on main 6575ce32508bd7908444241f3719aa33aaf77f90 using the installed OpenCode SDK 1.15.13 against a local HTTP fixture that withheld command response headers. On Node 26.12.1 with bundled undici 8.12.2, session.command rejected after 300.814 seconds with TypeError: fetch failed, caused by HeadersTimeoutError, code UND_ERR_HEADERS_TIMEOUT. This confirms the timeout mechanism independently of a model.
My alternate transport fix is fix(opencode): keep slash-command turns running past five minutes #27674, latest head e86c63ebceb008a2a49311a3d29d7ed14f1652c6. It disables header/body timeouts only for the synchronous command request. SDK HTTP fixtures completed after holding headers for 315 seconds on Node 24.22.1 / undici 7.30.1 and Node 26.12.1 / undici 8.12.2. Ordinary asynchronous prompts and in-flight AbortSignal cancellation passed on both. Node 22.20.0 / undici 6.21.2 passed the same compatibility checks without the long hold.
Those runtime checks used the actual helper while it was uncommitted, checked its source hash before and after execution, and matched the committed helper: SHA-256 7d8fa563a548a66b23b342bf0549d1fc2037f867d0f75a5ee1dccae3c22db940.
The focused real-HTTP regression passes with the helper and fails with plain fetch, caused by UND_ERR_HEADERS_TIMEOUT. It also verifies cancellation. The existing native-command user-receipt admission test passes.
vp run check:changed passed with 50 tests across seven files on the latest head. Required CI passed on the preceding head 0895b1d7c3128fb2f8fd913d566b9aa298755b15. CodeRabbit subsequently found that the added warning exposed the raw SDK error; e86c63ebce replaces it with the fixed runtime error tag, keeps the original runtime cause chain, and passed the local checks again. The transport helper is unchanged. The review thread is resolved; updated CI was still running at handoff.
Not verified by me: your admission-based fix, the reporter's VPS, Electron, real proxy routing, or a stalled response body. The reproduction and results are also summarized in my PR body. Temporary fixture servers and files have been cleaned up. I am stopping work on my branch following your takeover.
The OpenCode command error handler now skips turn failure finalization when the command receipt is complete. A regression test checks that the turn completes after a request timeout and subsequent busy and idle session events.

Explanation
Issue #27664 requires admitted command turns to remain active until OpenCode reports the result. The change adds Deferred.isDoneUnsafe(receipt) || before the existing command-request failure finalization in packages/provider-opencode/src/server/adapter.ts. The event stream can therefore complete the turn after admission. Pre-admission failures and admission timeout handling remain unchanged. The new test sends the user receipt, rejects session.command with HeadersTimeoutError, then sends session.idle and expects completed. The existing failure path still passes the request cause to makeProviderFailure, and the post-admission path no longer creates a request-failure item.

Full details: Description check
Explanation
The description follows the repository template well. It has a clear Problem section with reproduction steps, a Change section that explains the admission-based fix in submitPrompt, and a Verification section with a test, runtime transcripts, a mechanism check, and a note on what was not checked. The Scope and approval section links issue #27664 and says it was triaged via the automated issue flow. That is a weak point, because the template asks for the triaged issue or a discussion with explicit maintainer approval, or a justification for a small obvious fix. The link is present, but the description does not show a maintainer approval comment, and the 'Triaged via the automated issue flow' wording does not show that. The Takeover and Evidence sections add a large amount of material, most of it hosted on external links that a reviewer cannot easily verify. The description also states that the same work was done by a different model and harness, which the template asks for. Overall the description is mostly complete, so the check passes, but the approval evidence should be strengthened.

Recover request failures after command receipt completion.
If the receipt completes and the SDK request then rejects before the admission race settles, Line 2994 skips finalizeTurn, but Effect.tapError still propagates the rejection. Fiber.join(request) can win Effect.raceFirst with that failure. The startTurn error handler then fails a command that OpenCode has already admitted. Recover the request failure after a completed receipt before racing the request against the receipt. Keep failures before receipt completion unchanged. (effect.website)

Preserve the request failure detail for pre-admission command failures.
When session.command fails before its receipt completes, makeProviderFailure receives the OpenCodeRuntimeError wrapper and no explicit message. The emitted failure is then the generic Provider turn failed., even though OpenCodeRuntimeError.detail holds the request failure. Pass that detail as the explicit message. makeProviderFailure applies the existing redaction to explicit messages before it builds the failure contract.

Deferred.isDoneUnsafe(receipt) proves only that OpenCode published the user receipt. OpenCode can then fail while persisting setup state. The session.command endpoint returns an HTTP error before runner startup and does not publish session.error. The current guard suppresses that rejection, so the turn can hang.
Use the nested response status to distinguish a definite OpenCode response failure from a transport failure. Keep suppression for admitted errors without an HTTP status, including UND_ERR_HEADERS_TIMEOUT.
