fix(claude): approval cards close when Claude withdraws the request - #27320
When Claude Code withdraws a permission request it no longer needs (control_cancel_request), the thread keeps showing "Approval needed". This happens, for example, when a PermissionRequest hook already allowed the command. The card stays until someone clicks it, and the click does nothing because the tool already ran. The Claude adapter stopped waiting when the SDK's abort signal fired, but never reported the request as closed, so it stayed pending.
When the wait in canUseTool ends because Claude aborted it rather than because the user answered, the adapter now emits the request, its node and its turn item as cancelled. That is the status the server already gives native questions left open when a turn ends. AskUserQuestion requests wait the same way and get the same handling.
The adapter checks whether the user's answer reached it, not only whether the signal aborted, so a request that got an answer stays resolved.
Scope and approval
Fixes the bug in #27282. The reporter reproduced the CLI side headlessly: can_use_tool, then control_cancel_request for the same request 70 ms later, then the tool result, while the T3 projection row stayed pending.
Verification
New test in ClaudeAdapterV2.test.ts: opens an approval in an approval-required turn, aborts the SDK signal, ends the turn, then checks that the latest request, node and turn item updates are cancelled. Without the fix it fails with expected 'pending' to equal 'cancelled'.
The existing approval tests in that file still pass (8 in the run).
Server typecheck, lint and format pass.
Not checked: a live Claude Code session with a PermissionRequest hook.

Macroscope's review found this PR approvable — Withdrawn Claude requests now mark their approval or input cards as cancelled instead of leaving them open. Existing permission decisions and approval defaults are unchanged.

Walkthrough
The Claude adapter now cancels runtime requests, execution nodes, and turn items when Claude withdraws an unanswered permission or user-input request. Regression tests cover Bash approval and AskUserQuestion cancellation.
When an answer or decision remains pending after waiting ends, the adapter marks the runtime request, execution node, and turn item as cancelled. Tests check that withdrawn Bash approval and AskUserQuestion callbacks deny and that their artifacts are cancelled.
No actionable issue remains in the supplied review scope; the change is ready for normal merge checks.

Issue #27282 is closed and supplies historical context only. No active directly linked coding requirements apply. The reviewed changes still implement the reported behavior: withdrawn Bash and AskUserQuestion requests mark the runtime request, node, and turn item as cancelled, while answered requests remain resolved. Focused regression coverage verifies both request types.

- Line 9699: Update the withdrawn-request handling around Deferred.isDone and
cancelWithdrawnRequest so cancellation is emitted before terminal finalization
can stop event ingestion, even when a result arrives while the aborted approval
callback is still running. Add a regression case that queues the result
immediately after aborting the callback and before joining its fiber, and verify
the cancellation event is preserved.
- Line 7110: Update cancelWithdrawnRequest to apply the pending-cancellation
guard to approval artifacts as well, persisting cancellation only when the
request, its node, and its turn item are still pending; preserve any response
already committed by dispatchRuntimeRequestRespond.
