Problem

When Claude Code withdraws a permission request it sent T3, the approval card stays open for good. Claude Code runs PermissionRequest hooks (and Auto mode's classifier) at the same moment it sends can_use_tool, then cancels T3's request when one of them answers first. The adapter returns, but the request stays pending and its node and approval item stay waiting. Approve then fails with No pending Claude runtime request, and until a server restart the pending request keeps the thread from being settled, snoozed, resumed after a usage limit or idle-compacted.

Repro and evidence from the desktop app (0.1.2, Claude Code 2.1.308) are in #24690. In short: with a hook that allows in bypass mode, a Full access thread running F=$(cat dir.txt); rm -f $F/* runs the command at once and leaves a waiting card. Pressing Approve on two such cards produced 20 failed respondToRuntimeRequest spans.

Change

After awaitClaudeApprovalDecision, if the decision Deferred is still not done, the abort signal won and nobody answered. The adapter then emits cancelled for the request, node and approval turn item, the same way #22803 withdraws an aborted question. A person's answer, including Cancel, is left alone: the decision is done, so no withdrawal is emitted over it.

Scope and approval

Fixes #24690. It is the approval half of #15517: #22803 covers aborted AskUserQuestion requests and leaves approvals unchanged. #22023 (Stop) and #22982 (run end, tools that never ran) don't reach this case, because here the turn completes and the tool runs. The change is one branch in the Claude V2 adapter's approval path, plus tests.

Verification

• ClaudeAdapterV2.test.ts, new "approvals Claude withdraws" cases:
  • closes the card when the CLI cancels its request: a Full access Bash approval whose signal aborts produces request:pending -> cancelled, node:waiting -> cancelled and item:waiting -> cancelled. Against main's adapter it fails with only the waiting/pending updates.
  • leaves a card the user answered to the user's decision: a person answers Cancel, then the request is aborted. The adapter emits no withdrawal. It fails if the withdrawal keys on a cancel decision instead of an unanswered Deferred.
  • Both read the harness events behind a second card's receipt. Events reach the harness in order, so the assertion doesn't wait on timing.
• The full ClaudeAdapterV2.test.ts file passes (214 tests) on 90c455ff8f. Server typecheck passes, and format is clean on both files.
• On the SDK side (@anthropic-ai/claude-agent-sdk 0.3.276, Claude Code 2.1.308, a hook that allows and a canUseTool that waits 15 s): Claude Code called canUseTool and aborted it after 15 ms, then ran the command. That abort is the path this change handles.
• Not checked: a live run of this branch in a client. I observed the stuck card on 0.1.2 and covered the fix with the adapter tests above.

Closing in favor of #24767, which makes the same adapter change for #24690. It also claims the decision with Deferred.succeed before cancelling, which closes the abort-window race CodeRabbit raised here, and extends the EventSink guard so committed answers survive pending delivery.
