ApprovabilityVerdict: Needs Human Approval Macroscope's review found this PR requires human approval — This change alters shared provider-session lifecycle and cross-thread run/delegated-task settlement across several production orchestration layers. The concurrency and durable event-state coordination make the runtime impact broader than a small isolated bug fix. You can add or adjust custom eligibility rules. Learn more. |

Track and cascade run-owned work apps/server/src/orchestration-v2/RunExecutionService.ts |

On ingestion failure, the service interrupts an unfinished root turn or terminalizes remaining open work if the run was already finalized. Tests cover tool work, completed roots, and superseded writes. |
Finalize delegated tasks apps/server/src/orchestration-v2/Orchestrator.ts, apps/server/src/mcp/DelegatedTaskProviderExit.integration.test.ts |
The orchestrator passes owned turn items to cascade terminalization. The integration test checks child-run, subagent, tool, and delegated-task states for both root-turn outcomes. |

This change makes Codex runs and delegated tasks settle when the shared app-server exits, and it keeps per-thread session state correct across releases. No actionable merge-blocking risk remains in the reviewed changes. An app-server that hangs without exiting is not covered here, and the author tracks it separately.

Preserve release status for threads outside the replacement.
If the old session served threads A and B, and the replacement opens only for A, replaced suppresses the old release event for both threads. B has no replacement attachment event, so B’s projection remains ready after its session closes. Suppress the old status update only for threads attached to the replacement; write a release event for the others.

Prevent an older release from overwriting a completed replacement release.
If the old scope close stalls, a replacement can open and release before the old close times out. The map is then empty, so this check writes the old release status after the newer release status. For example, an old manual_shutdown can replace the newer session’s runtime_error with stopped. Fence status writes by session generation or the projected session’s identity, not only by whether a session is currently live.

Provider exit can leave tools and native subagents running after their root finishes, strand a delegated task, or let a delayed session release overwrite a replacement's state. This repairs the lifecycle failures reported in #17045 and the Windows reproduction. Codex app-server termination detection is already on the updated base.
The repair tracks release ownership per thread, hands ownership over inside EventSink's committed transaction boundary, and persists per-thread session snapshots so shared sessions retain each thread's state. Cleanup consults those snapshots before removing a worktree. Provider-stream failure also settles owned tools/native work and delegated results, while preserving completed root results, partial output and current-attempt fences.
