fix(server): recover remote delegated launches after source restart - #30249
If the source server restarts after the destination accepts a delegated launch but before the response is recorded, the destination thread can keep running without a source task to follow or stop. This continues #20937 on main after its feature-branch base was deleted and that PR closed unmerged.
Summary
delegate_task to a linked environment
- launch destination thread
record source task
+ record launch attempt+ launch destination thread with the original replay key+ attach destination receipt
startup
+ replay pending launches with the same key and link identity
follow attached tasks
Change
Record resolved launch inputs before the outbound request and retain the accepted task ID across ambiguous failures. Recovery reuses the original request namespace, key, modes and OAuth-grant identity. Replacing a link cannot start another child for an uncertain launch. Cancellation before a request is sent does not launch; cancellation after uncertain acceptance reconciles the original child and stops it.
The destination thread ID is absent until attachment. Task tools and web/mobile navigation handle that pending state, show Launching, and retain actionable launch errors. Existing attached tasks remain readable. No new MCP tool or question/approval forwarding is added.
Scope and approval
Fixes#17051, filed by maintainer @juliusmarminge with this record-before-launch, attach, and startup-replay design. Its triage comment identifies a durability/recovery bug and leaves the changed task-result and restart semantics for manual review. That comment is not represented as implementation or merge approval. The prerequisite #16719 is now merged. This contribution addresses that one recovery window; local approval notices and remote parent-handled questions remain separate.
Evidence
The lost-response test fails on the tested main base with no source task after destination acceptance, and passes with recovery enabled.
The following captures compare the same synthetic pending task in the isolated web client, first with the main-base web components, then with this fix. The right-hand lineage row changes from Running to Launching, with navigation disabled until attachment. This is rendering evidence, not a physical two-host launch reproduction. The fixture retains an older synthetic failure notice in its timeline.

Verification
Base tested: 8657eecb28bc8c767510cc6b9d852b628047c604 on main.
vp test run src/peer/RemoteDelegation.test.ts --maxWorkers=1 from apps/server: 43 cases pass. Coverage includes a destination accepting while its response is withheld, rebuilding the source follower without an active parent run, attaching the original destination thread, and rejecting reuse after the link grant changes.
The same lost-response regression with RemoteDelegation.ts restored from that main base fails at the first persistence assertion: expected [] to have a length of 1 but got +0. Restoring the fix passes the suite.
Mobile subagent presentation: 6 cases pass. Total focused cases: 138.
Scoped server, web, mobile and contracts type checks pass. Web needed a higher memory budget after the first bounded run timed out.
Formatting and changed-file lint pass without findings. vp run check:changed --concurrency 1 passes format, lint, the files/dependencies knip pass, and GraphQL generation. Its export pass reaches the 768 MiB Node heap limit, and its 35-package typecheck reaches the 900-second bound. The remaining broader test stage was interrupted. A separate export retry at 1536 MiB also failed to finish within approximately 15 minutes. These broader checks are incomplete, not green.
The launch schema is private to its contracts module; final contracts types, formatting and lint pass after removing its unused export.
No paid MCP model-use eval ran because this checkout has no eval API key. No model-facing instructions or input tool descriptions changed.
The remote integration test uses real HTTP/MCP/OAuth and database-backed source state with a controlled destination launch service. It does not prove physical two-host, real-provider restart, Connect, desktop-shell or native-mobile acceptance.
Merge Danger
Door: Two-way for code, with a persisted-state compatibility caveat.
Blast Radius: Remote delegation.
The fix adds optional persisted launch inputs and allows a task to exist before its destination thread ID is known. Both sides continue to enforce link access and caller modes. Rolling back while pending intents exist cannot be assumed to preserve recovery; let those tasks settle before rolling back.

Remote delegation records launch details before contacting the destination. The server reconciles pending launches and attaches accepted threads after retries or restarts. Task results and client rows represent launches without a thread ID. Thread launch errors preserve typed missing-project details for MCP responses.

Remote task contracts retain launch inputs and support launch and attachment commands. The orchestrator validates these commands, attaches accepted threads, and hydrates the parent transcript before dispatch. Tests cover attachment after cancellation, mismatched identities, and repeated Stop commands.
Peer link identity checks apps/server/src/peer/PeerLinks.ts, apps/server/src/peer/PeerMcpClient.ts, apps/server/src/peer/PeerForwarding.ts, apps/server/src/peer/PeerMcpClient.test.ts
Stored peer links expose a launch identity derived from the secret name. Peer calls and thread waits can forward an expected identity, which PeerMcpClient checks before making the MCP request.
Launch, replay, and cancellation reconciliation apps/server/src/peer/RemoteDelegation.ts, apps/server/src/peer/RemoteDelegation.test.ts
Remote delegation records launch inputs before sending, replays pending launches, and attaches accepted threads and runs. Restart tests cover withheld responses, cancellation, stops, permission changes, and revoked or replaced links.
Remote task results include environment and label when the thread ID is absent, and include launch errors when present. Web and mobile rows show launch status or errors and do not navigate until a thread ID is available.
Thread launch wraps a missing project in a typed ProjectNotFoundError. The project MCP handler maps that cause to invalid_request; tests distinguish it from a project lookup failure, which maps to orchestration_error. A launch replay test verifies reuse of the same thread, message, and run IDs.

Explanation
Open directly linked issue #17051 has coding requirements. The PR records a pending remote task before launch and stores the request data, link identity, and launch state. It keeps remoteChild.threadId unset until attachment. It launches and replays with the original key. The attach command records the destination thread and run without an active parent run. Startup retries pending launches and attaches the receipt. Web and mobile code handles the pending state, shows “Launching,” and blocks navigation until attachment. Remote delegation tests cover lost responses, restart recovery, cancellation, duplicate prevention, and link changes. Focused tests and type checks reported by the PR support these changes.

Explanation
The changed code stays connected to #17051. Contract changes represent pending launch state and attach commands. Peer-link identity and forwarding changes prevent replay against a replaced link. Orchestration changes implement launch, attachment, cancellation, stop reconciliation, and launch errors. Thread management and project error classification support reliable replay outcomes. Web and mobile changes present and control pending remote tasks. Tests cover the new recovery and presentation behavior. No unrelated tool, approval, question-forwarding, or model-facing change is shown.

Add launchIdentity to the resolve fixture in PeerMcpClient.test.ts.
ResolvedPeerLink.link now has type StoredPeerLink, which requires launchIdentity. The resolve fixture at apps/server/src/peer/PeerMcpClient.test.ts:136-146 builds a link literal without that field, so the typecheck fails there. This PR changed PeerLinks.ts but not the test file, so the fixture was not updated with the type change.
Other Layer.mock(PeerLinks.PeerLinks)({}) fixtures pass no resolve or get implementation, so they need no change for this type. The fixture change alone fixes the reported error. The import-style concern is separate and is not covered here.

Update the resolve fixture in PeerMcpClient.test.ts so its ResolvedPeerLink.link
literal includes the required launchIdentity field from StoredPeerLink. Leave
mock fixtures that do not implement resolve or get unchanged.
Crypto.Crypto.digest is already used at Line 273. The new Node import and diagnostic suppression are therefore unnecessary. They also make launchIdentity bypass the Effect crypto service.

As per coding guidelines, reserve local nodeBuiltinImport exceptions for cases where Effect lacks an equivalent API. As per path instructions, this file must follow docs/internals/effect-services.md.

The outside-diff fixture finding is fixed in 0ae7779: the PeerMcpClient.test.ts resolve fixture now supplies launchIdentity, and scoped types including that test pass.
I am retaining the Node SHA-256 implementation rather than applying the crypto nitpick. CI on 5d62cde reports TS2339 for Crypto.digest; the peer-link shard also fails at that call with TypeError: crypto.digest is not a function. Restoring the suggested call would restore that verified failure. Current main uses the separate Digest.Digest API for PKCE, rather than the proposed Crypto.Crypto.digest. The Node implementation preserves the same SHA-256 fingerprint and passes the affected tests.
Merged current main (merge commit, no rebase). One test-file conflict in handlers.test.ts; both sides' tests are kept.
Fixed a type error the merge exposed: main's peer-link relabel helper returned a plain PeerLink, dropping launchIdentity from resolved links. It now keeps the stored link type.
Mobile grouped subagent avatars now use the live remoteChild, per the open CodeRabbit finding.
vp run check:changed passes on a testbox: format, lint, knip, typecheck (36 packages), 21 test files / 351 tests, react compiler.
The verification below was written by gpt-6.1-sol against an earlier head.
Verified in a real dev environment
Verified on Linux with repo Playwright and /usr/bin/google-chrome, isolated app-created projects, two real T3 servers linked through OAuth, and Codex gpt-6.1-sol through the local proxy. A forwarding proxy withheld the real launch response only after the destination accepted it. No fake provider responses or seeded fixtures were used.
On github/main (3ee55151e0), killing and restarting the source lost the delegated task: zero source subagents, while the accepted destination child finished.
On this PR, the pending task survived restart and attached the same accepted destination thread and run. The child returned REMOTE_RECOVERY_OK; the final recovery rerun returned FINAL_RECOVERY_OK.
Ordinary delegation returned NORMAL_REMOTE_OK. Navigating from the parent to the recovered child and back worked.
UI Stop exposed a missing remote-child guard. After the scoped fix, Stop during an unconfirmed launch followed by restart interrupted the same destination child and recorded Stopped with its parent.
Forgetting the peer link after acceptance produced the expected warning that work may still be running. Fixed web/mobile selection of live launch metadata so the warning remains visible. A repeated-stop race also dropped this warning; the existing regression case failed before the final fix and passed afterward while preserving the newer pending stop.
Final head: 8c478af8997c3455624afaf5f5884a654658406c. vp run check:changed 8657eecb28bc8c767510cc6b9d852b628047c604 passed through blacksmith-check: format, lint, knip, GraphQL, 35 package typechecks, 331 tests, and 9 selected compiler checks. Final independent source review found no remaining supported findings.
GitHub does not render this external video inline; direct playback and seeking were checked in Chrome.
Not verified
Native mobile and desktop shells, macOS/Windows, Connect/tunnel, and other providers were not exercised on this Linux VM. The exact repeated-stop timing was covered by the existing regression test, not recreated through timed UI clicks. Missing-project and replaced-link variants were covered by automated checks rather than separate real-provider runs.
Recovery deduplicates the persisted request identity. A resumed provider independently reissued a tool call from a new provider-session namespace and created another task, as the existing namespace semantics permit. This verification does not claim deduplication across separate provider sessions.

Problem
If the source server restarts after the destination accepts a delegated launch but before the response is recorded, the destination thread can keep running without a source task to follow or stop. This continues #20937 on main after its feature-branch base was deleted and that PR closed unmerged.
Summary
Fixes #17051, filed by maintainer @juliusmarminge with this record-before-launch, attach, and startup-replay design.
