feat: a running turn with no agent activity for 10 minutes is flagged - #27254
A frozen agent looks exactly like a busy one. During the incident every thread said Working for hours while nothing arrived from any provider.
Fix
Server.ProviderSessionManager keeps one in-memory timestamp per running turn:
set when the turn starts;
touched on every provider event, including command output deltas (events for provider threads it doesn't track, such as native subagents, touch every turn in the session);
touched only for its own thread when a shared-session event names one (plans, runtime requests), so a busy sibling cannot hide a frozen thread;
dropped at turn.terminal, when the thread is detached from its session (Force stop on a shared session), or when the session is released.
Quiet runs. A Stream.shared check every 30 s, which runs only while a client listens, emits the threads quiet for 10 minutes or more, and only when that set changes. Cost per event is one Map write; no database, projection or shell writes.
Transport. Clients opt in with quietRuns: true (capability quietRuns) and get quietRunsUpdated on the config stream, like memory pressure.
Waiting is not quiet. A thread waiting on an approval, a question, or another thread (t3_thread_wait) is never flagged; the shared selectors decide this for web and mobile, lists and detail views alike.
Wording. Neutral: "No activity for 12m — A long tool may still be running, or the agent may be stuck." Tools legitimately run that long, so nothing is called stalled or broken.
Web.
The thread shows that line with Stop and Force stop.
The sidebar row says "No activity 12m" in the warning tone instead of "Working".
Each row selects only its own entry, so an update re-renders just that row.
Mobile.
The floating status shows "No activity for 12m"; its action offers Stop or Force stop.
The thread list row is marked the same way.
Verification
Tests:
The session manager flags a started turn after 10 minutes of test-clock time and clears it once the provider sends an event.
The config projection carries quiet runs across a reconnect snapshot and clears them when empty.
A sibling's plan update in a shared session doesn't refresh a quiet thread, and detaching the thread drops it.
The selectors ignore threads waiting on approval, input, a subagent request, or another thread.
vp run check:changed passes for the stack.
Evidence (web)
For the capture only, the threshold was lowered to 30 s in an uncommitted patch. The Replay agent was frozen with SIGSTOP; nothing else was simulated.
Before (main): a frozen agent is indistinguishable from a working one
After: thread and sidebar both flag the quiet turn
Limits
Mobile is not captured; Metro on nucbox-1 was ruled out while it hosts the live server.
The timestamps are in memory, so after a server restart a turn is flagged only once it has been quiet for 10 minutes again.

Macroscope's review found this PR requires human approval — Web and mobile now automatically flag turns after ten minutes without provider activity and offer Stop/Force stop, without automatically terminating anything. Human review must confirm this default warning threshold and presentation, since legitimate long-running tools can also trigger it.

The server tracks active runs with no provider events for at least 10 minutes and publishes quiet-run updates to subscribed clients. Web and mobile interfaces display quiet status and elapsed time, and provide Stop and, when permitted, Force stop actions.
Tracks activity for turns and compactions, emits quiet-run results, and removes activity records when runs or sessions end. Tests cover late terminal events and quiet runs in shared sessions.
Gates quiet-run updates by capability and subscription option. Client runtime retains updates for replay and configuration snapshots, removes empty quiet-run lists, and provides thread-level quiet-run selectors. Web and mobile connections enable the subscription.

Do not refresh activity for a stale terminal event.
startEventPump touches the provider-thread entry before markIdle checks runOrdinal. A late terminal can therefore refresh the newer run's lastEventMs, which can clear its quiet state or delay its next warning.

Match quiet state to the active run.
When a queued follow-up starts before the next quietRunsUpdated tick, the thread-only selector keeps the completed run. Web and mobile can then show “No activity” and expose actions that target the completed run. The server rejects that stale ID, so the newer run is not stopped, but the status and actions are misleading. Pass the current runtime.activeRunId to the quiet selectors and require it to match the quiet entry. Keep the quiet notice while the follow-up is only queued.

@coderabbitai Both outside-diff findings from your review of 6611a2c were valid. They're fixed in 78f0e40, each with a test that failed first.
Stale terminal event refreshing the newer run (ProviderSessionManager.ts). eventActivityTarget now passes the runOrdinal from turn.terminal, and touchRunActivity refreshes a tracked run only when the ordinals match. A late terminal from run 1 still marks its own turn idle, but no longer refreshes run 2's lastEventMs. Test: the existing "flags a quiet startTurn/compactThread, despite an older run's late terminal" case now delivers the stale terminal one minute before the threshold. Before the fix, run 2 was never flagged and the test timed out; now it's flagged on time.
Match quiet state to the active run (threadExecution.ts). quietRunSince and quietRunId now take the thread's runtime.activeRunId and return an entry only when its run is still the active one. A follow-up that starts before the next 30 s refresh therefore hides the notice and its actions right away. A follow-up that is only queued leaves the quiet run active, so the notice stays. Every caller passes the active run: web thread view, web sidebar row, mobile thread screen and mobile thread list. The selectors still return primitives, so each row re-renders only when its own value changes. A new selector test covers the matching run, a newer run, and no active run.

apps/mobile/src/features/threads/thread-list-v2-items.tsx:1321-1323 Minor · Functional Correctness · Quick win
Include quiet status in both row accessibility labels.
RowPressable forwards the explicit label to React Native Pressable. The child status has no accessibility label, so VoiceOver and TalkBack announce only the thread title and queued-message state. Include No activity when quietSince !== null.
Suggested fix
accessibilityLabel={
- props.hasQueuedMessages ? `${thread.title}, messages queued to send` : thread.title+ [+ thread.title,+ quietSince !== null ? QUIET_STATUS_LABEL.label : null,+ props.hasQueuedMessages ? "messages queued to send" : null,+ ]+ .filter((part): part is string => part !== null)+ .join(", ")
}
Apply this change to both RowPressable variants.

Show the quiet duration in folder rows.
When a folder or folder-multiline row becomes quiet, its status slot renders only topStatus.label. It shows “No activity” without the elapsed time. Only card rows render WorkingDuration from quietSince. Add the duration to the folder status slot so those rows show how long the run has been quiet.

@juliusmarminge Thanks. Source inspection confirms both fixes:
turn.terminal carries runOrdinal into activity tracking. A mismatched ordinal does not refresh the newer run.
Both quiet-run selectors match runtime.activeRunId. All four web and mobile caller groups pass that ID.
The regression tests cover the stale terminal for both startTurn and compactThread, plus selector behavior for matching, newer, and absent active runs. The active-run check also preserves the notice when a follow-up is only queued.
These changes address both findings. I inspected the test code but did not run the tests.

Do not use the latest run as the match target: a queued follow-up can be newer while the quiet run remains active, and must not hide its notice or change its Stop or Force stop target.

# T3 Code
## Hit every surface
The most common defect in this repo is a change that works on the path you tested and is missing everywhere else. Before calling frontend work done, walk this list and say which entries applied:
- **Agents.** A capability a user can trigger is usually one an agent should reach through MCP tools, and scheduled tasks run the same paths. That only works when it is a service method, not handler code.
Outside-diff Minor, "Include quiet status in both row accessibility labels" (thread-list-v2-items.tsx): valid, fixed in e7fc4c3. Both RowPressable variants now share one label built from the title, "No activity" while quietSince !== null, and "messages queued to send", joined with ", ". VoiceOver and TalkBack therefore announce the quiet state. There's no test: this is label wiring in a React Native row with no existing test harness, and check:changed (typecheck, lint, react compiler) passes.
Unverified additional comment, "Show the quiet duration in folder rows" (SidebarThreadRow.tsx): leaving it as is. Folder rows have no status line, so the status takes the time slot and shows only its label for every status, "Working" included. Adding a duration only for quiet rows would make them inconsistent with the rest, and card rows already show it.
(The GitCafe→GitHub mirror is lagging behind a GitHub API rate limit, so e7fc4c3 may take a few minutes to appear here.)
Accessibility labels: The inspected mirror still has the old labels in both RowPressable variants. Verification of e7fc4c3deb is therefore inconclusive. Please retry verification after the mirror catches up. I do not request a label-wiring test; AGENTS.md excludes tests of props rendered to static markup.
Folder-row duration: Source inspection supports your explanation. Folder rows show the status label in the time slot, while card rows include the duration. I withdraw the duration suggestion. No change is needed.
I did not run checks or verify VoiceOver or TalkBack behavior.
A thread waiting on an approval, a question, a subagent request or t3_thread_wait is no longer flagged as quiet. Before, web chat and both detail views could offer Stop or Force stop on a turn that was waiting on the user. The rule now lives in the shared selectors.
plan.updated and runtime_request.updated refresh only the thread they name, so a busy sibling in a shared session can't keep a frozen thread unflagged.
Detaching a thread from a shared session drops its run tracking. Force stop on a shared session gets no terminal event, so that entry used to stay in every quiet-runs payload.
A frozen agent looks exactly like a busy one: the thread says Working for hours
while nothing arrives from the provider.
The session manager now records when each running turn last heard from its
provider (one in-memory timestamp per event). While a client listens, it checks
every 30 s and pushes the threads quiet for 10 minutes or more on the config
stream. The thread shows "No activity for N" with Stop and Force stop, and the
web sidebar and mobile thread list mark the row. The wording stays neutral:
tools can legitimately run that long.
