feat: ADHD mode answers waiting agents one card at a time, on web and mobile - #27095
When many agents run at once, the sidebar makes you keep checking which ones are waiting on you. ADHD mode (beta) shows only the threads that need you, one card at a time, so you can clear them from the keyboard (or by tapping, on mobile) without watching anything.
Each card shows what the agent did and what it needs, with numbered options. Agents blocked on an approval or a question come first, then failed and finished threads in your sidebar project order. Working, snoozed and settled threads stay out. On finished or failed cards, the text generation model (the one that writes thread titles and commit messages) suggests the next prompt, so E sends it and R lets you tweak it first.
Keys: ↑/↓ pick an option, E accept, R reply in your own words, Backspace snooze 15 min, Esc dismiss (settle), 1–9 jump to a card, O open the thread. On multi-select questions, Space picks options and E submits them. A subagent waiting on you gets its own card.
Off by default. Turn it on in Settings → Labs → ADHD mode, then open it from the command palette or with Mod+Alt+J.
New orchestration.suggestNextPrompts RPC (operate scope) backed by NextPromptSuggestionService and a generateNextPrompts method on the text generation providers. Drivers without text generation return their usual unsupported error.
Mobile (iOS and Android) gets the same queue: Settings → Labs → ADHD mode, then the bell button in the thread list toolbar or the command palette. Tap an option to answer; Snooze 15 min, Dismiss and Open are buttons. Replies go through the mobile outbox, like the composer's sends. The queue logic moved to client-runtime (state/adhd-queue) so web and mobile share it, each passing in its own thread list status rules.
On web, every key also has a click target: the queue pips (with the thread's title as a tooltip), accept, reply/send, snooze, dismiss and open.

Validation
In a dev server with synthetic projects and Replay threads, ran through every key. E answered a question and the next card took its place, Backspace snoozed (8 → 7), Esc settled (7 → 6), 1–3 jumped. R + edit + Enter sent "Finish the search provider fixture plan, using UI flexibility over strict schemas. Keep it short." to the thread (checked with t3 thread show). Both Mod+Alt+J and the palette open /adhd.
The real text generation model returned thread-specific suggestions for Done threads.
vp run check:changed after rebasing on main: format, lint, knip, typecheck (32 packages) and react compiler pass; 722 tests pass. The browser test files didn't run because this machine has no Playwright browser.
New tests: queue ordering and exclusions (adhdQueue.logic.test.ts), and the suggestion service, including the failed-run flag (ThreadTitleRegenerationService.test.ts), including rolled-back runs and the failed run's error.
New eval evals/next-prompts (finished, failed with an error, failed before any reply, finished-PR control). Ran 3× on gpt-6-luna; all 12 samples met their rubrics.
Not checked live: an approval card (the Replay approval scenario auto-approved in full-access mode), a multi-select question, and a subagent card.
Mobile was not run on a device: this machine has no simulator access, so there are no mobile screenshots yet. It typechecks and lints, and its code follows the existing mobile request, snooze, settle and outbox paths.
After adding mobile: vp run check:changed passes format, lint, knip, typecheck (36 packages) and react compiler; 75 of 79 test files pass, and the other 4 are browser tests that can't launch Playwright here.

This change adds ADHD mode on web and mobile, with queues for threads needing attention and cards for approvals, input, failed runs, and completed runs. It also adds next-prompt generation through provider implementations and an orchestration RPC.
Adds the provider contract, prompt builder, and sanitization that returns up to three prompts. Adds provider implementations or unsupported-operation responses and wires the operation through server text generation. Updates test doubles for the new operation.
Adds the RPC contract, authorization scope, and WebSocket handler. The suggestion service filters streaming and rolled-back conversation content, selects the latest non-rolled-back run, and passes thread and project context to text generation.
Build and validate the shared ADHD queue packages/client-runtime/src/state/adhdQueue.ts, packages/client-runtime/package.json, apps/web/src/components/adhdMode/adhdQueue.logic.ts, apps/web/src/components/adhdMode/adhdQueue.logic.test.ts
Adds shared queue classification and sorting. The web adapter supplies sidebar status and unseen-completion checks. Tests cover queue ordering and exclusions.
Add the web ADHD mode view and entry points apps/web/src/components/adhdMode/*, apps/web/src/routes/_chat.adhd.tsx, apps/web/src/routeTree.gen.ts, apps/web/src/components/CommandPalette.tsx, apps/web/src/components/CommandPaletteLookups.ts, apps/web/src/components/settings/*, apps/web/src/shortcuts/commandPaletteCommands.ts, packages/contracts/src/settings.ts, packages/contracts/src/keybindings.ts, packages/shared/src/keybindings.ts, docs/user/thread-sidebar.md
Adds the web route and card interface, including queue navigation, responses, suggested prompts, snoozing, dismissal, and keyboard controls. Adds the Labs setting, command-palette action, and keyboard shortcut.
Add mobile ADHD mode and preferences apps/mobile/src/features/adhd/*, apps/mobile/src/features/home/ThreadListToolbar.tsx, apps/mobile/src/features/keyboard/CommandPalette.tsx, apps/mobile/src/features/settings/ThreadSettingsSections.tsx, apps/mobile/src/features/workspace/WorkspaceToolbar.ios.tsx, apps/mobile/src/persistence/mobile-preferences.ts, apps/mobile/src/Stack.tsx
Adds the mobile route and queue cards, including approval, input, and prompt actions. Stores the feature setting in mobile preferences and adds settings, toolbar, and command-palette entry points.
Add next-prompt evaluation cases and runner evals/README.md, evals/next-prompts/*, evals/package.json
Adds evaluation cases, a CLI runner that generates prompts and can compare them with a baseline, and usage documentation.

Ignore auto-repeated key events in the card keydown handler.
The handler does not check event.repeat. If the user holds E or Enter for longer than the key-repeat delay, the first event answers the card. Repeated events then reach the next card, which mounts after the answer. This can approve a second request that the user has not read. It can also auto-answer every question in a multi-question prompt with its recommended option. Held Backspace and Escape repeat snooze and dismiss in the same way.
Ignore repeated events for every key except the arrow keys, which should keep repeating.

Clarification: the UI consistency comment applies to the raw footer action buttons below the reply textarea; it was inadvertently anchored to the textarea handler.
Correction to the inline suggestion in packages/contracts/src/orchestrationV2.ts: it is anchored one line too early. Keep threadId: ThreadId unchanged; replace only line 3928 (cause: Schema.optional(Schema.Defect()),) with cause: Schema.Defect(),. Do not apply the suggestion to line 3927.

Switching queue cards discards unfinished questionnaire answers, so returning to the thread restarts at question 1 with the user's edits lost. The key={threadKeyOf(current)} unmounts AdhdCard, and its local draft and progress.answers state is not preserved; keep that state outside the keyed card or warn before leaving an unfinished questionnaire.

Leaving this as is for the beta. Web has the same behavior: progress on a multi-question card lives in that card until the last answer is sent. Leaving a card is an explicit jump (a number key or a pip), nothing is sent half-done, and answering again takes a few taps. Keeping per-card drafts across jumps would mean a draft store shared by both clients, which is more than this PR needs. Happy to add it as a follow-up if it trips people up.

Adds a new ADHD-mode route on web and mobile, a card-by-card agent-attention workflow, model-generated next-prompt suggestions and a default Mod+Alt+J shortcut. These are new product behavior and workflow, beyond configuring existing thread controls. The PR supplies no prior maintainer direction/scope approval. Its linked bug reports concern narrower pending-question visibility/delivery problems; their full discussions do not approve this feature. The related Inputs/Next input proposal #27064 likewise has only contributor support, not maintainer approval.
Obtain explicit maintainer approval of the ADHD mode direction, suggestion-generation behavior and intended scope in an Ideas discussion, then link that approval and align the PR with it.
