perf(server): sweep stale attachments without blocking the event loop - #26809
The pending-attachment sweep used `readdirSync`/`statSync`/`unlinkSync` and ran inside the upload-URL request handler (and at startup), stalling the event loop that serves every WebSocket client while it walked a potentially large attachments directory.
`sweepStalePendingAttachments` is now async (`fs.promises`), and both callers await it via `Effect.promise`. Existing sweep tests updated; no behavior change.
Not measured with a benchmark. Typecheck could not run locally (missing `tsc` binary in this checkout); CI will cover it.
Macroscope's review found this PR approvable — Attachment cleanup now uses asynchronous filesystem operations to avoid blocking the event loop. Existing deletion eligibility, retention thresholds, and caller sequencing remain unchanged.
You can add or adjust custom eligibility rules. Learn more.
The stale pending-attachment sweep now returns a promise and uses promise-based filesystem operations. Server callers and tests adapt to the asynchronous API.
The sweep uses promise-based operations to read the directory, inspect file modification times, and delete stale files. Directory-read failures still return { deleted: 0 }, and per-file failures are skipped. The test awaits the sweep and checks that four files were deleted.

Server setup and upload URL issuance wrap the sweep promise in Effect.promise. Server setup captures the current time before calling the sweep. The guest service test also wraps the call in Effect.promise.

Use the injected FileSystem service for the stale-attachment sweep.
sweepStalePendingAttachments is persistence code and currently uses NodeFS.promises directly. AttachmentUpload.issueAttachmentUploadUrl is a service operation, not an application-boundary adapter, but calls the sweep through Effect.promise. Implement the sweep with FileSystem.FileSystem and yield it directly from both callers. Preserve the current best-effort error handling and deletion behavior.

the verification rule requires observed results, not just updated tests and an expectation that ci will cover the change. the description gives no executed test command/result or runtime sweep result for the asynchronous cleanup and its upload/startup callers. it also expressly has no measurement for the event-loop claim.
report a focused sweep-test command and results for stale-file deletion and retention, and check the caller behavior after making the sweep asynchronous. if retaining a performance claim, provide evidence matched to that claim; otherwise limit it to the demonstrated asynchronous implementation. state any unavailable checks and request reconsideration.
