fix(server): honor worktree prefixes when automatic naming fails - #25957
With Worktree branch naming → Static prefix set to my-prefix, a new worktree still starts on t3/<8-hex>. If automatic naming is unavailable or fails, that temporary name becomes permanent and ignores the saved preference.
Apply the resolved project prefix when creating the temporary branch, using the existing branch-name formatter. Check each prefix namespace for Git branch collisions and flatten the temporary name when necessary. Keep these branches eligible for the later automatic rename.
This is a focused correction to an existing setting: no new settings, workflow, or notifications. Naming failures remain nonblocking. If settings cannot be read, worktree creation retains the original temporary name.
Scope and approval
This qualifies for the small, focused obvious-bug exception in CONTRIBUTING.md: a saved static prefix should apply even when the text-generation provider cannot supply a descriptive name. Only the launch service and its existing test file change. Issue-number, keyword, and issue-timeline searches found no existing PR for this report before opening this one.
Verification
Added five regression cases first; all five failed on the original code because the branch remained t3/<8-hex>.
The launch-service tests now observe my-prefix/<8-hex> on naming failure, my-prefix-<8-hex> when a plain my-prefix branch blocks the namespace, and team-topic-<8-hex> when either nested namespace is blocked. They also cover an empty prefix and project overrides.
A settings-secret read failure regression failed before the follow-up fix (failed instead of starting); it now verifies that worktree creation and setup continue on t3/<8-hex> without an error item.
A successful-generation control verifies that a prefixed temporary branch is still renamed to my-prefix/fix-parser.
vp run check:changed upstream/main passed: formatting, lint, both knip checks, server GraphQL generation, all three affected typechecks, and 48 launch-service tests.
Tests use the real in-memory orchestration with mocked Git and text-generation boundaries. No live provider or platform-specific desktop/mobile run was performed; there is no client UI change.
Implemented and verified by GPT-6.1 Sol through the Codex harness in T3 Code.
Macroscope's review found this PR approvable — This is a localized server bug fix that preserves the existing default branch naming while honoring configured static prefixes when automatic naming fails. Collision handling and fallback rename behavior are covered by targeted regression tests.

Walkthrough
Generated temporary worktree branches now use the configured static prefix when naming mode is static. The service checks slash-delimited branch ancestors for collisions and allows generated temporary branches to be renamed after an initial message. Tests cover generation failures, settings-read failure, and successful naming.

Summary
Temporary branch naming and fallback apps/server/src/orchestration-v2/ThreadLaunchService.ts, apps/server/src/orchestration-v2/ThreadLaunchService.test.ts
Static naming mode applies the configured prefix to generated temporary branches. Before worktree creation, collision checks examine slash-delimited ancestors and adjust the name when a local branch exists. Generated temporary branches are eligible for initial-message renaming. Tests cover generation failures, settings-read failure, and successful naming.
The supplied evidence identifies no remaining issue that should block merging after normal checks.

Explanation
The title "fix(server): honor worktree prefixes when automatic naming fails" matches the main change. The diff makes temporary worktree branches use the configured static branch prefix when automatic naming fails or is unavailable. It follows the conventional commit format from the template and names the server component. The title is specific and short, so a teammate scanning history can tell what changed.
Full details: Description check
Explanation
The description follows the template's main sections. The Problem section states the bug and gives reproduction context (static prefix set to my-prefix, temporary t3/<8-hex> name persists, linked issue #25871). The Change section explains the fix. The Scope and approval section cites the CONTRIBUTING.md obvious-bug exception and explains why it applies. The Verification section lists focused tests, their outcomes, and what was not checked (no live provider run, no platform-specific run). Minor gap: the template asks for an explicit approval link or comment for the exception, and the description gives a rationale and a linked issue but no maintainer approval comment. The description also reports 48 launch-service tests, while the objectives report 47. The author should confirm the count.

Explanation
Issue [#25871] requires static prefixes on temporary and fallback worktree branches, project overrides, namespace-collision handling, and nonblocking naming failures without user-facing error notifications. ThreadLaunchService resolves project settings before temporary branch creation, formats the temporary branch with formatGeneratedBranchName, checks slash-delimited prefix namespaces, flattens blocked generated names, and preserves later rename eligibility. The added tests cover generation failure, settings-read failure, collisions, empty prefixes, project overrides, successful rename, setup continuation, and absence of error turn items.

Explanation
The pull request changes only ThreadLaunchService.ts and its existing test file. The implementation and tests directly support issue [#25871]. No unrelated settings, workflow, notification, or client changes are present.

ran vp i and the real background dev server on github/main (32ecd71726fee16f335484c1f95cbce126d461f6) in a separate worktree, stopped it, then ran the exact pr head with an isolated t3 home. drove the web app using the repo's playwright and /usr/bin/google-chrome.
created a project through the app pointing at a real git repository. saved the static prefix through settings, selected new worktree, and sent the same task through codex gpt-6.1-sol: create slug.mjs, run node slug.mjs Hello World, report its branch, and leave the work uncommitted. no mocks, fixtures, replay provider, or demo data were used.
naming failure: selected a nonexistent custom model only for source control writing. the real naming request failed with model_not_found, while chat stayed on gpt-6.1-sol. main retained t3/3fcb0501 despite the saved my-prefix setting. the pr retained my-prefix/86a88fba. both turns completed and returned hello-world.
successful naming: restored the source control writer to gpt-6.1-sol. the next pr worktree changed from my-prefix/02142c34 to my-prefix/node-slug-cli. its turn completed and returned hello-world.
namespace collision: created a real branch named collision-prefix, saved that prefix, and repeated the real naming failure. the pr created collision-prefix-a3aad232 and completed the turn without a git namespace error.
confirmed the actual branch names with git and ran each generated cli directly. all four returned hello-world. all 48 existing ThreadLaunchService.test.ts tests passed through blacksmith-check. the stored pr status reports 7 successful checks, 0 failing, and 0 pending. no source changes were made, so check:changed was not required for a fix.
native ios/android, the electron desktop shell, macos/windows, remote relay/tunnel connections, and providers other than codex were not exercised on this linux web run. project overrides, empty/nested prefixes, and settings-secret read failures were covered by the existing focused tests, but were not separately exercised through the real app. the main client needed a reload to display its completed turn; the pr turns displayed their results live. uploaded video playback and seeking were checked in chrome. git.cafe renders this external host's media as links; the published comment and the actual uploaded files were reopened and checked.
