fix(server): renaming a thread no longer re-sends phone alerts - #25617
Renaming a thread sent other devices a fresh "Agent finished" (or approval/input) push. The rename republished the thread's activity state with a new updatedAt, and the relay's notification-only paths treat a new time as a new alert. For approval/input they don't check the time at all.
Fix, on both sides:
Server (AgentAwarenessRelay): when the phase hasn't changed since the last publish, the republished state keeps the published updatedAt. A rename, project rename or model switch still updates the Live Activity card, but it no longer looks like a new completion. This also keeps Android's on-device alert_id dedupe stable.
Relay: upsert returns the row it replaced. If the new publish has the same phase, the publisher skips iOS notification pushes: the notification-only path and the Live Activity fallbacks (no activity token, and the companion push sent with live_activity_end). Live Activity alerts already compare phases, so they are unchanged. The relay check also covers server restarts and relinks, which clear the server's memory of what it published.
Proof: three new tests, one in each of AgentAwarenessRelay.test.ts, AgentActivityPublisher.test.ts and ApnsDeliveries.test.ts. Each fails on main and passes with the fix, for example:
A same-phase publish never alerts on iOS notification paths. A real new alert always follows a phase change (for example running → completed). The only exception: the relay misses the in-between publish entirely while still receiving the later one. In that same case the server today already skips a repeat that hasn't changed.
A Live Activity row's 2-hour/24-hour expiry is now counted from when its phase started, not from the latest rename.
A rename republished the thread's awareness state with a fresh updatedAt,
and the relay's notification paths treat a fresh time as a new alert.
The server now keeps the last published time while the phase is unchanged,
and the relay skips notification pushes when a publish repeats the stored
row's phase. Live Activity alerts already compare phases and are unchanged.

Macroscope's review found this PR approvable — This is a focused cross-layer bug fix that preserves Live Activity updates while suppressing duplicate phone-alert paths, with targeted tests and no schema or deployment changes. An unresolved High finding identifies a retry scenario that may permanently suppress a notification after the activity row is persisted, so that correctness risk remains important to address separately.

Walkthrough
The relay preserves updatedAt when an activity remains in the same phase. Persistence returns the stored state for delivery. APNS jobs use stable identifiers for eligible notifications, and delivery checks compare queued metadata with current activity data.

Eligible push notifications use a hash of target and activity identity fields as the job ID; other notifications use the existing UUID fallback. Delivery checks compare sanitized display metadata as well as phase and timestamp. Tests cover stable IDs after title changes and rejection of stale metadata.
pingdotgg/t3code#29358 — The issue reports repeated publishes of null state after threads settle. This change records null tombstones in published-state tracking.
pingdotgg/t3code#11938 — The issue reports a repeated completion alert after changing the model on a finished thread. This change preserves timestamps when the activity phase remains unchanged.

Renaming a thread no longer re-sends alerts. However, a thread that finishes a new run while a publish is still in progress, and is renamed at the same time, can lose its completion alert on the phone. This is a narrow edge case that should be addressed or explicitly accepted before merge.

Full details: Title check
Explanation
The title names the main change: renaming a thread no longer re-sends phone alerts. The changeset matches it. The server keeps the published updatedAt for same-phase publishes, and the relay skips iOS notification pushes for same-phase publishes. The title uses the conventional commit format and is specific.
Full details: Description check
Explanation
The description follows the template. It gives the problem, the change on both server and relay sides, and the scope link (Fixes #25582 with linked issue objectives). It also gives verification: three new tests, a before/after trace, and a vp run check:changed pass. The author states the trade-offs. The template asks for a maintainer approval comment in Scope and approval. The description links the issue but does not cite an explicit maintainer approval of direction. The template also asks for the model and harness, and the description includes 'Claude Opus 5.5 in Claude Code.' The PR objectives say live APNs/FCM, physical-device, and native client behavior is unverified. The description does not say this, so the verification section is incomplete.
Resolution
Optionally, add the maintainer approval comment or triaged issue reference that the Scope and approval section asks for. Also state in Verification that live APNs/FCM delivery, physical-device behavior, and native iOS/Android behavior were not verified.
Full details: Out of Scope Changes check
Explanation
The changes remain related to [#25582]. They preserve the phase timestamp, use the persisted state for delivery, prevent stale Live Activity metadata from overwriting a rename, and stabilize notification job identity. The added test updates cover these behaviors. No unrelated product behavior is demonstrated.
Full details: Linked Issues check
Explanation
Issue [#25582] requires no push or in-app notification after a same-phase rename for completed and attention states. The server preserves updatedAt, and the iOS publisher skips same-phase notification paths. The Android path remains incomplete. AgentActivityPublisher passes the stored state to FcmDeliveries.enqueue, but FcmDeliveries.process can still call androidAlertForState when the phase and timestamp match. androidAlertForState calls shouldAlertForActivity, which allows waiting_for_approval and waiting_for_input without a freshness check. A notification-only Android target can therefore receive alert: true for a same-phase rename. Android alert_id deduplication does not establish that no push is sent.
Resolution
Suppress Android alert generation when the published phase is unchanged from the previously stored state for the same environment and thread. Preserve Live Activity data updates. Add a regression test for a notification-only Android target in an attention phase after a same-phase rename, and assert that no alert is sent.

Restore the replaced activity row when delivery fails after the upsert, so
the environment's retry of the same state still alerts.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
thanks @torztomasz. i kept your server phase timestamp fix and both original commits, merged main, and updated this pr rather than replacing it.
the relay now preserves the phase timestamp in one atomic upsert and returns the stored state. the separate prior-row read, global rollback, and repeatsAlert plumbing are removed.
apns notification jobs reuse an identity per event and target, including its token and routing. existing durable delivery claims prevent a successful target from ringing again after another target's enqueue fails. legacy payloads keep random ids.
android receives the stored timestamp too, preserving its alert identity across restart snapshots. queued live activity updates also check current metadata so an old job cannot undo a rename.

blacksmith vp run check:changed passed: 95 tests, affected typechecks, lint, format, knip, and graphql checks.
the rename regression fails with current main's server code. new queue identity, persisted-state fanout, and stale-card regressions failed before their fixes and pass afterward. no new test files.
runtime postgres check via pglite and the actual rows service: rename retains 12:00, the next phase advances to 15:00, and eight overlapping same-phase upserts retain 15:00 in both json and the scalar column.
both independent reviews approve this head. all seven reported github checks are green and the pr is mergeable. coderabbit's latest automatic review was rate-limited; no new bot findings were posted.
unverified: live apns/fcm delivery and physical phone behavior; no relay deployment or native client run. this changes server/relay behavior, with no client layout change.
@torztomasz, a separate existing limitation remains: apns delivery claims treat transport/5xx failures as completed attempts. this change reuses that delivery policy; improving transport retries belongs in a separate fix. android's existing native dedup history is bounded, and phase-only dedup still assumes the intervening phase reaches the relay.

the original duplicate phone alert could not be reproduced on main, and suppression could not be verified on this pr. this vm has no authenticated t3 connect environment link or registered phone. both servers remained behind the activity publication gate. local client notifications are a different path and would not prove this fix.
relay timestamp retention, apns/fcm delivery, live activity updates, restart/relink catch-up, per-device retry deduplication, and stale queued-card rejection were not exercised against a real relay/device. native ios/android and desktop clients were not run.
the next check requires a signed-in t3 connect environment and registered phone: complete a real turn, rename its thread without another phone alert, then run another turn and confirm a fresh completion alert. repeat for approval/input states and reconnect catch-up.
no code changes or pushes. existing reported checks: 7 successful, 0 failing, 0 pending; no checks rerun in this pass. these runtime checks establish neighboring behavior only, so this pass is recorded as unverifiable for the requested alert fix.
Adding a concrete regression case for the Android bounded-history limitation already noted here.
At 742748eb92f71cc7a9c50ee17403ef9a5b8b710a, this case requires no server restart or relink:
Enable device notifications and disable ongoing activity for both the Android device and environment.
Receive an alert for a thread waiting for input.
Receive 64 other distinct alerts while the original waiting row remains within its 24-hour TTL.
With the phone backgrounded, rename the original thread.
FCM's notification-only path sends the old alert ID inside a fresh updated_at=now envelope. The native receiver accepts that envelope and can post again because the ID has been evicted. The existing alertHistoryEvictsOnlyTheOldestEntryAfterCapacity test explicitly expects an evicted ID to become eligible again.
This supplies a specific follow-up test case for the acknowledged limitation. It is source-verified, including preference, TTL, freshness, registration and foreground guards; I have not exercised Android or real FCM delivery.

Preserve the run identity across coalesced publishes.
The relay coalesces queued thread IDs. If a thread moves from completed to running and back to completed while a publish is in progress, the queued publish can read only the final completed snapshot. If the title changed, the snapshot is published, but withPublishedPhaseTime copies the old completion's updatedAt.
The notification-only APNs path passes that state to shouldAlertForActivity, which rejects terminal timestamps older than two minutes. A genuinely new completion can therefore lose its phone alert.
Preserve a run or turn occurrence identity through the relay and use it for duplicate detection and completion alerts. Add a regression test for the coalesced transition.
