fix(server): recover Codex after external account switches - #24856
Codex threads can recover after an external codex login changes accounts. Once Codex reports the account mismatch, the next requested turn replaces the idle app-server and resumes the same native conversation.
Codex caches CLI credentials, and account/read(refreshToken: true) refuses a different account. Recovery uses the existing session-release path and the optional hasStaleCredentials hook also proposed in #16849. Busy turns, compaction, pending requests, and background work prevent replacement. Failed prompts are never automatically replayed. Managed authentication is unchanged.
Isolated runtime evidence and probes cover native Codex 0.156.1 and 0.164.0. A full T3 adapter/session-manager/effect-worker run with pinned 0.156.1 reproduced the mismatch, replaced the process, preserved conversation history, and reused the replacement for a new thread. It made 3 successful inference requests and 1 rejected request; both child processes were reaped. All 131 focused tests and vp run check:changed pass.
Limits: credentials and endpoints were synthetic. Real ChatGPT sign-in, Ubuntu/systemd, and client UI were not tested.
Important
Automatic auth replacement runs once until a native turn succeeds. Shared work can delay it; repeated failure retains manual sign-in/restart guidance.

Walkthrough
The Codex adapter identifies eligible account-mismatch failures and exposes stale-credential state. The session manager uses that state to replace an idle runtime while preventing replacement during active turns, operations, or background work.

Summary
Detect and expose stale credentials packages/provider-core/src/server/ProviderAdapter.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts
The runtime contract adds an optional stale-credential check. The Codex adapter records eligible account-mismatch failures, adds recovery details to failure messages, and exposes the check for unmanaged runtimes. Tests cover unauthorized messages and runtime configurations.
The session manager tracks active operations, checks credentials before reusing a runtime, and releases a stale runtime only when turns, operations, and background work are inactive. Wrapped operations reject calls against replaced runtimes. Tests cover turns, compaction, resume, and interrupted attachment.
pingdotgg/t3code#23720 — The change replaces an idle shared Codex runtime after an account mismatch and resumes the persisted native thread.

Explanation
The title follows the conventional commit format and names the main change: Codex recovery after external account switches. The raw summary shows the adapter, session manager, and runtime changes that implement this recovery path, so the title matches the changeset.
Full details: Description check
Explanation
The description follows the template's core sections. It states the problem (Codex caches CLI credentials and refuses a different account after an external login), the change (idle app-server replacement and resume through the existing session-release path with a hasStaleCredentials hook), the scope (it links issue #23720 and describes limits), and the verification (isolated probes, a full adapter/session-manager run, and 131 focused tests). The Scope and approval section lacks an explicit maintainer approval comment or discussion link, and the template asks for one. The description also does not explicitly state the reproduction environment beyond the Codex versions. These gaps are non-critical because the description is mostly complete.

Explanation
The changes satisfy the coding requirements in #23720. CodexAdapterV2 detects the specific unauthorized account-switch message for unmanaged Codex sessions and exposes hasStaleCredentials. ProviderSessionManager replaces the stale runtime on the next idle open, preserves the persisted native thread, and blocks replacement during active turns, pending operations, resume, and background work. Wrapped startTurn and compactThread operations reject stale runtime handles, so failed prompts are not replayed. The regression tests cover stale detection and delayed replacement for turns and compaction.
Full details: Out of Scope Changes check
Explanation
The changed files remain within #23720. They add account-mismatch detection, stale-runtime replacement, operation-safety controls, the shared runtime hook, recovery guidance, and focused regression tests. These changes directly support recovery after external codex login without changing managed authentication. No unrelated change is demonstrated.
