fix(codex): say why the Codex app-server exited - #23938
mkantautas wants to merge 5 commits into
Conversation
The Codex client drained the app-server's stderr and threw it away, so a launcher that dies before initialize showed only "Codex App Server process exited with code 1". A one-click npm update that skips @openai/codex-<platform> leaves exactly that state, and the launcher's own explanation never reached the provider banner. The client now keeps the last 8 KiB of stderr. When the process exits, CodexAppServerProcessExitedError carries the line that names the failure (the last error-labelled line, else the last line), stripped of terminal controls and capped at 512 characters, and appends it to its message.
ApprovabilityVerdict: Needs Human Approval Macroscope's review found this PR requires human approval — This is a focused Codex process-diagnostics fix that captures a bounded stderr excerpt and adds it to process-exit errors, with an additive optional field and dedicated tests. It also introduces a line-level ESLint suppression for the ANSI escape regex, which requires human review. You can add or adjust custom eligibility rules. Learn more. |

Collect and normalize stderr packages/effect-codex-app-server/src/_internal/stdio.ts, packages/effect-codex-app-server/src/_internal/stdio.test.ts |
The collector retains the latest 8,192 characters. Excerpt extraction removes terminal control characters, selects an error-labelled line or the last nonempty line, and limits the excerpt to 512 characters. Tests cover normalization, selection, and limits. |
Attach stderr to termination errors packages/effect-codex-app-server/src/errors.ts, packages/effect-codex-app-server/src/_internal/stdio.ts, packages/effect-codex-app-server/src/_internal/stdio.test.ts |
CodexAppServerProcessExitedError accepts optional stderr. Termination handling adds a nonempty excerpt after a successful status read. Tests cover nonzero exits with stderr and blank stderr. |
Wire stderr collection into the client packages/effect-codex-app-server/src/client.ts, packages/effect-codex-app-server/src/client.test.ts |
The client passes a collected stderr tail to termination handling instead of draining stderr with a background stream fiber. A client test checks the error for a missing-dependency process failure. |

Keep raw stderr out of the exported error.
If Codex prints a credential, signed URL, or command argument on the selected line, this code places that value in stderr and the user-facing error message. Removing terminal escapes and limiting the line to 512 characters does not remove sensitive values. Keep the raw tail private; expose only a safe, structured diagnostic. As per path instructions, error attributes “must not expose raw output, arbitrary defect text, credentials, or command arguments.”

The excerpt follows AcpProcessExitedError in packages/effect-acp/src/errors.ts, which carries a stderr attribute and appends it to its message for the same reason: the launcher's failure line is what tells a user how to fix the install (#16160). This keeps one line, strips terminal escapes and caps it at 512 characters. Leaving this open for the maintainers to decide whether Codex gets the same exception as ACP.

@mkantautas Understood. The ACP precedent explains why the launcher’s failure line is useful here. The excerpt limits control size and formatting, but do not redact sensitive values.
I’ll leave the finding open for maintainers to decide whether Codex should receive the same exception as ACP. I’m not requesting another change before that decision.

Note Written by thanks @mkantautas. the bounded stderr tail and error-line selection preserve the repair guidance that the old client discarded. i kept your commits and merged current main into this branch. i replaced the custom terminal-escape regex and lint suppression with node's built-in stripper, moved the real subprocess regression to the live clock, and fixed a timeout race found during review: stderr now drains before stdout eof reaches the protocol, so an inherited open stderr cannot discard a known exit code. the collector reuses its drain fiber. all changes stay in the original five files. verification on
unverified: windows/macos, packaged desktop, native mobile, and mid-session process death. the final coderabbit review was skipped by the organization's quota; its check is successful, and both independent reviews covered the final change. stderr excerpts retain cli-provided text and are not credential-redacted; the existing diagnostic-display policy question remains for maria's review. |

Problem
When the Codex app-server exits before
initialize, T3 Code shows only "Codex App Server process exited with code 1". The client drains the child's stderr and drops it, so the reason the launcher printed never reaches the provider card, the model picker tooltip or a failed turn.I hit this on Nightly on 2026-10-23: my
@openai/codex0.164.0 install had lost@openai/codex-darwin-arm64, the state #16160 describes after a one-click update. The launcher said exactly what to do:T3 Code showed "Codex app-server provider probe failed: Codex App Server process exited with code 1." on the card, and every Codex thread failed with "Codex App Server process exited with code 1".
Change
makeChildProcessClientkeeps the last 8 KiB of the app-server's stderr instead of draining it into nothing. Draining still happens, so a chatty stderr cannot block the protocol (the existing 512 KiB test still passes).makeTerminationErrorwaits up to 250 ms for stderr to finish, then puts one line onCodexAppServerProcessExitedError.stderr: the last error-labelled line (Error: …,TypeError: …,Error [ERR_…]: …,error: …,fatal: …), otherwise the last non-empty line. Terminal escapes are stripped, whitespace collapsed, and the line capped at 512 characters.A Node launcher prints its thrown error between a source excerpt and the stack frames, so the first stderr line would be
file:///…/codex.js:107. That is why the excerpt picks the error-labelled line rather than the first one. Thestderrfield mirrorsAcpProcessExitedError. #22878 does the same for the Claude version probe.Scope and approval
Refs #16160, a triaged bug reproduced by a maintainer. The triage lists "Pass the child's stderr through to
CodexAppServerProcessExitedError, so messages like 'Missing optional dependency' show up in the UI" as one fix area. This PR does only that part. It changes no contract, setting or UI code. The card and tooltips already render the error message.Verification
