ApprovabilityVerdict: Not approved Macroscope's review found this PR not approvable — This production server refactor changes pull-request discovery from shell/history reads to a new projection-store query path across startup, refresh, and backfill flows. Focused tests cover the intended metadata semantics, but the altered runtime data-access boundary and operational impact warrant human review. Notes:

- Line 5649: Update the discovery read pipeline at Effect.mapError to select
thread_id alongside each payload and map decode failures per row with that
thread’s context; map SQL read failures to the read-error type rather than
ProjectionStoreSetupError, without wrapping the entire pipeline in one generic
error.

Note Written by Thanks for working on PR discovery performance. This is a 353-line query refactor, with synthetic snapshot timings rather than a reproduced user-visible failure. Right now we are only taking focused bug fixes from outside contributors, under the limited review capacity described in Read this first. Please preserve the performance report in #18618 and request reconsideration with a focused fix for a reproduced user-visible failure. |
PR discovery builds thread shells even though it only needs thread metadata. That counts old tool history and reads run/input state that discovery never uses, on both startup and refresh.
Read active thread metadata directly through the existing orchestrator. Keep startup backfill, settled-thread filtering, single-thread scope, legacy PR links and the sequence guard against stale updates. Decode the payloads after the read transaction ends.
On 500 threads with 50,000 completed tool items, the returned discovery metadata matches the old shell metadata:
About 63% less time for this fixture's snapshot read, not for GitHub calls or the whole app.
Reproduction scripts.
Validation: 59 focused tests pass, plus server typecheck, clean targeted lint and formatting on the M4 mini. The earlier isolated client loaded a failed 50,000-tool fixture and passed search, settled-thread and PR navigation without page errors.
Test cleanup: removed 4 of 7 added cases (57%)—duplicate in-memory examples and fabricated diagnostic failures. Three SQL runtime cases remain: legacy links/explicit unlinking, active/backfill filtering, and avoiding history reads. Existing integrated metadata/sequence assertions remain. Deliberate mutations losing legacy links, including settled threads, adding a history read, or returning a stale sequence all fail the surviving checks. Types/schemas cover structure, not these query semantics. No production code, dependencies or runner configuration changed. Independent Codex review of the deletion slice found no actionable regression.
No client, provider or wire changes; the shared server path covers web, desktop, mobile and remote connections. The installed app was not updated, and verification servers used separate state.
Fixes #18618.
