Problem

Suppose a Claude instance has its own homePath, so its sessions live under that home's CLAUDE_CONFIG_DIR instead of ~/.claude. After a server restart, resuming one of that instance's subagents through SendMessage can't recover the subagent. The adapter asks the SDK's getSubagentMessages for the tool call that launched the subagent. The SDK resolves session files from CLAUDE_CONFIG_DIR in the server process, so it looks in the wrong home and returns []. The lookup silently yields null, and the resumed subagent's output isn't reattached to its original child thread.

The cause is the same as in #15598, which fixes forkSession (#15040 has the same fork fix as its "Bug 19"). This PR fixes the other in-process history read in the V2 runner, subagentLaunchToolUseId, which neither PR touches.

Change

  • ClaudeAgentSdkQueryRunner.subagentLaunchToolUseId now takes the instance environment. When its CLAUDE_CONFIG_DIR differs from the server's, the lookup runs in the existing Claude history worker under that environment, the way the V1 adapter ran its history reads before feat(orchestrator): introduce new orchestrator #2829. Otherwise it still calls the SDK in-process, so behavior for the default home is unchanged.
  • The history worker accepts getSubagentMessages. The subagent id and limit travel in the options JSON, so the __claude-history CLI arguments stay the same.
  • The adapter passes adapterOptions.environment to the lookup.

The worker invocation matches the one in #15598 and #15040. Whichever lands second can share one helper, and I'm happy to rebase onto either.

Scope and approval

This is a small, focused fix for an obvious bug, with no behavior change for instances that use the default home. It's a regression from #2829: the V1 adapter ran history reads in the instance environment, and the V2 runner reads in-process.

Verification

  • New test ClaudeAdapterV2 subagent launch lookup > reads a subagent stored under the instance's own Claude home. It writes a parent session, a subagent transcript, and its .meta.json sidecar under a temp Claude home. Then it calls the real layerQueryRunner with CLAUDE_CONFIG_DIR pointing there.
    • Without the fix it fails: expected null to equal 'toolu_launch'.
    • With the fix it passes.
  • vp test run src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts src/provider/ProviderInstanceRegistry.test.ts: 2 files, 166 tests passed.
  • tsc --noEmit for apps/server passed with no diagnostics on changed lines. vp lint and vp fmt --check on the touched files are clean, apart from the existing no-unused-vars 'layer' warning.
  • I built the npm bundle with vp pack and ran dist/claude-history-worker.mjs getSubagentMessages … against a copied real subagent transcript. With the instance's CLAUDE_CONFIG_DIR it returned the subagent's messages; with the server environment it returned []. The adapter code lands in dist/binCli-*.mjs, next to the worker, so ./claude-history-worker.mjs resolves.
  • I ran into the root cause with a CLI-proxy Claude instance whose homePath is ~/.claude-proxy.

Not checked: the single-executable (__claude-history) path, and a full restart-then-SendMessage flow in the desktop app. The test covers the runner directly.

Model and harness: Claude Opus 5.5 in Claude Code (running inside T3 Code).

🤖 Generated with Claude Code

…ir own home

After a server restart, resuming a subagent through SendMessage looks up
the tool call that launched it with the SDK's getSubagentMessages. The SDK
reads session files from CLAUDE_CONFIG_DIR in the server process, so an
instance with its own homePath always got an empty result and the
resumed subagent was not reattached to its original child thread.

Run the lookup in the Claude history worker under the instance's
environment when its CLAUDE_CONFIG_DIR differs from the server's, as the
V1 adapter did for its history reads.

Approvability

Verdict: Approved at e2d337c

Macroscope's review found this PR approvable — This is a localized fix for resumed Claude subagents using an instance-specific Claude home, with unchanged default-home behavior and direct subprocess/integration coverage. It does not alter product defaults, schemas, deployment, or static-analysis settings.

📝 Walkthrough

Walkthrough

The Claude history worker now supports subagent message retrieval. When the configured CLAUDE_CONFIG_DIR differs from the host’s, the query runner runs the lookup in a child process with the instance environment. The adapter passes its environment to the lookup.

Merge Risk | ⚪ Minimal · up to e2d33

Merge Risk: ⚪ Minimal · up to e2d33

The instance-home lookup change is ready to merge after normal checks; no actionable merge-blocking risk remains.

Security Architecture Review

Security architecture risk: 🔵 Low · up to e2d33

This is a narrowly scoped correction to instance-specific history reads. The inspected changes preserve session and thread ownership and add no remote interface. No security defect was established, but packaged-runtime behavior and underlying history isolation remain incompletely verified.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The changed operation exposes local history-derived launch attribution to the existing recovery path for instances with a different configured Claude home. The inspected command construction adds no network destination or privilege escalation mechanism. Effective access to other local histories still depends on process permissions, configuration authority, and SDK path resolution, which were not fully established.

Trust Boundaries and Controls

  • observed — The executable and worker entrypoint are selected by host services and fixed source paths. Session, agent, directory, and options values travel as arguments and JSON rather than interpolated shell commands. The worker requires a session ID and a nonempty agent ID for subagent retrieval and dispatches only explicitly supported methods.
  • observed — Recovery requires a tracked tool call and a task absent from both current-turn and session registries. A successful lookup changes launch attribution, not the context-derived owning thread or provider instance. The base already accepted string parent identifiers without a nonempty constraint; that acceptance alone does not establish a PR-introduced vulnerability.

Resilience and Maintainability Implications

  • inferred — The transition inspection found existing registry guards, explicit task reopening, completion routing, and interruption finalization remain in place. The PR does not replace these lifecycle controls. Runtime interruption, full restart-to-completion recovery, and packaged worker behavior remain unverified rather than demonstrated safe.

1829-1912: 🎯 Functional Correctness | 🔵 Trivial | ⚡ Quick win

Add an adapter-path assertion for environment forwarding.

The new test calls ClaudeAgentSdkQueryRunner.subagentLaunchToolUseId directly. It does not execute the ClaudeAdapterV2 resume path that supplies adapterOptions.environment. An accidental removal of environment: adapterOptions.environment from the adapter call would therefore leave this test passing while breaking resumed-subagent lookup for instances with a separate Claude home.

Add a focused test that drives the adapter resume flow and asserts the lookup uses the instance environment. This covers the changed forwarding without duplicating the query runner's existing sidecar-reading assertion.

@coderabbitai The adapter-path nitpick is addressed in e2d337c. looks up a resumed subagent's launch in the instance's environment drives a SendMessage resume through ClaudeAdapterV2 for an instance with its own CLAUDE_CONFIG_DIR and asserts that the lookup receives it. With environment: adapterOptions.environment removed it fails: expected [ undefined ] to deeply equal [ '/instance/claude-home' ]. The stderr comment is answered inline.

@coderdevang Thanks for the clarification.

  • The adapter-path concern is addressed. I checked that the test drives a SendMessage resume and asserts that the lookup receives /instance/claude-home. The adapter forwards adapterOptions.environment.
  • I withdraw the stderr finding. The lookup catches the error and logs only taskId. The Effect-services guidance permits the exact underlying value in cause, so my claim about this error reaching finalizeActiveTurnAfterQueryExit was incorrect.

Learning: In apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, the resumed-subagent launch lookup catches subagentLaunchToolUseId failures locally and logs only taskId under orchestration-v2.claude-subagent-launch-lookup-failed. This error does not propagate to finalizeActiveTurnAfterQueryExit. docs/internals/effect-services.md permits exact underlying failure values in cause while requiring bounded attributes and log annotations; retaining worker stderr in cause is not itself a violation.
