Builds on #16419.

Problem

Stop on a Claude turn sends the Claude Agent SDK an interrupt, then closes the query. Closing the
query is what stops the CLI and settles the turn. The close only ran when the interrupt succeeded. If
the SDK rejected the interrupt (the CLI answers the control request with an error, or the request
cannot be written), interruptTurn failed before the close. The CLI kept running, the turn stayed
running, and retries of the Stop effect took the same path.

#16419 bounds an unanswered interrupt so it falls through to the close. A rejected interrupt is the
other way the same step can fail, and it still skipped the close.

Fix

In ClaudeAdapterV2.interruptTurn, a rejected interrupt is logged
(orchestration-v2.claude-query-interrupt-failed) and Stop continues to the existing close. From
there it is the same path as an acknowledged or timed-out interrupt: the query ends and the turn
settles as interrupted once.

Why it qualifies

Very small, focused fix for an obvious bug: one call in interruptTurn. The happy path is unchanged.

How it was established and checked

New test in apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts:
closes the query when the native SDK rejects Stop. The SDK fake rejects the interrupt. The test
expects Stop to succeed, the query to be closed once, and one interrupted terminal. The wake
harness's interrupt option now also accepts a failing effect.

Before the fix (cd apps/server && vp test run src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts -t "Stop"):

× closes the query when the native SDK rejects Stop
ProviderAdapterInterruptError: Failed to interrupt claudeAgent provider turn provider-turn:provider:claudeAgent:native-turn:turn%3Arejected-stop.
Caused by: ClaudeAgentSdkQueryRunnerError: Claude Agent SDK query failed.
Caused by: Error: Claude Code rejected the interrupt

After the fix, the new test and the two Stop tests from #16419 pass. The whole file passes:
vp test run src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts.

Also run:

  • vp test run src/orchestration-v2/testkit/ClaudeReplayFixtures.integration.test.ts src/orchestration-v2/testkit/OrchestratorReplayFixtures.integration.test.ts src/orchestration-v2/ClaudeAutomaticDelivery.integration.test.ts src/orchestration-v2/testkit/OrchestratorReplayRecovery.integration.test.ts:
    pass.
  • cd apps/server && vp run typecheck: no errors in the changed files. The only errors are the
existing ones in src/process/externalLauncher.test.ts (Cannot find name 'testLayer'), which
    this change does not touch.
  • vp lint --report-unused-disable-directives apps/server/src/orchestration-v2/Adapters: no new
    findings.
  • vp fmt --check apps/server/src/orchestration-v2/Adapters: clean.

Not checked

I did not get a real Claude Code CLI to reject an interrupt. The test uses an SDK fake. The rejection
paths are from the SDK source (@anthropic-ai/claude-agent-sdk 0.3.276): interrupt() rejects on a
CLI error verdict, an unwritable or exited transport, and "Query closed before response received".

Found and fixed in a downstream fork (Scient), then ported to current main. Ported and verified with
Claude Opus 5.5 in Claude Code; the original fix was written with Codex (gpt-6.1-sol).

…ledgement

Stop waited for the Claude Agent SDK to acknowledge the interrupt with no
time limit. When the CLI never answered, the Stop request never finished and
the turn stayed running. The interrupt now waits at most 10 seconds, then
falls through to the existing close-and-settle path, which already bounds
itself and settles the turn as interrupted once.

…terrupt

Stop sent the Claude Agent SDK an interrupt and only closed the query after
it succeeded. When the SDK rejected the interrupt, Stop failed before the
close, so the CLI kept running and the turn stayed running. A rejected
interrupt is now logged and Stop falls through to the same close-and-settle
path as an acknowledged or timed-out one, which settles the turn as
interrupted once.

Approvability

Verdict: Approved at b161fe8

Macroscope's review found this PR approvable — This is a focused server bug fix that logs failed Claude interrupt requests and continues through the existing query-close and turn-settlement path. Regression tests cover acknowledged, unanswered, and rejected Stop scenarios, with no product-default or static-analysis configuration changes.

📝 Walkthrough

Walkthrough

Claude Stop handling now limits the wait for SDK interrupt completion to 10 seconds. If the interrupt fails or times out, the adapter continues to close the query. Tests cover acknowledgement, timeout, and rejection.

interruptTurn times out the SDK interrupt after 10 seconds and logs failures before continuing to close the query. Tests verify behavior when the SDK acknowledges, does not acknowledge, or rejects the interrupt. The test result-frame fixture supports user message UUIDs.

Change: Bug fix

Suggested reviewers: juliusmarminge

Merge Risk: 🔵 Low · up to b161f

Stop can still proceed to query closure after an interrupt timeout, but that timeout is not separately visible in warnings. This is a bounded operational concern rather than a demonstrated blocker.

Security Architecture Review

Security architecture risk: 🔵 Low · up to b161f

The change improves Stop handling without adding access or permissions. A residual risk remains when interruption and shutdown both fail: the turn can be reported as interrupted without confirmation that its process exited.

Retained concerns

  • Medium · security · inferred: Rejected or unanswered interrupts newly reach a fallback that ignores query-close errors and reports the turn as interrupted after the stream-closure wait expires. If closing also fails without terminating the CLI, execution could continue after the tracked turn is settled. The fallback existed before this PR, but its reachability now includes failed interrupt paths; actual SDK termination guarantees remain unconfirmed.
Security review details

Security Blast Radius

  • inferred — The relevant exposure is the lifetime of an already-running CLI query and its associated work relative to the application's stopped state. No new tool, credential, or caller authority is introduced in the production diff; the concern is residual execution under existing authority if cleanup fails.

Security Findings and Attack Paths

  • inferred — A conditional failure path remains: interrupt rejects or times out, close fails without ending the stream, and the closure wait expires. Head then settles the turn and releases query tracking without confirmed termination. Production occurrence and attacker inducibility were not established, so this is not a verified exploit finding.

Trust Boundaries and Controls

  • observed — Recorded execution-target validation, active provider-turn matching, and query-object ownership during stream exit are meaningful existing controls. They limit misrouting and stale-query cleanup, but do not prove termination when the fallback manufactures the closed signal.

Resilience and Maintainability Implications

  • observed — Interrupt failure no longer prevents a shutdown attempt, improving normal failure containment. The shutdown wrapper closes the prompt queue and calls the SDK close method, but the adapter ignores close errors; recovery guarantees therefore still depend on external shutdown behavior.

Log timed-out interrupts when operator visibility is required.

Effect.timeoutOption("10 seconds") returns Option.none, and this pipeline does not inspect that result. The interrupt-timeout path therefore emits no warning. The later claude-query-interrupt-timeout warning covers only the subsequent close wait.
