fix(server): restarts keep delegated tasks, queued threads, and stops intact - #15323

… intact - Delegated child tasks wait for their restart continuation instead of being reported cancelled; the parent gets the real result. Delegation recovery now runs after startup reconciliation. - A cut delegated-completion wake is not delivered twice, and wait-mode tasks wake the parent once the spawning turn is gone. - Parent recovery no longer cancels delegate_task rows or tells the parent they will not report back. - Continuation skips queued runs when picking the cut run, so a queued follow-up or wake no longer disables it. - A run the user stopped, or a cut /compact or /logout, is not continued. - Mid-turn and chained continuations carry the lost background work note. - One failing thread no longer aborts shutdown preparation for the rest. - Scheduled tasks and usage-limit resumes wait for startup recovery. - Held queues are no longer reread on every boot.

…led continuations - Recovery orders runs by when they ended, so a queued run resumed after a continuation is still continued, and its background roster run is loaded. - A child stays held while any later continuation is pending, task_status reports it as working, and a continuation that fails for good settles it. - settled_only siblings join a delivery once their spawning run ended. - The replay harness runs delegated task recovery like startup does, and held-queue selection tolerates malformed payloads.

with "continue threads after restarts" enabled, restart recovery could report a delegated child's interrupted result as final, lose background-work notes, run queued work too early, or continue work the user had stopped.

blacksmith: server typecheck and targeted lint passed; 85 focused orchestration/mcp tests passed, followed by 44 projection/recovery tests and both recovery benchmark tests after the query change, then 43 note/continuation/provider-start tests after the attempt-delivery fix. existing tests cover repeated restarts, failed continuations, interrupted deliveries, per-provider ownership, queue ordering and explicit stops.
the real web client and codex provider were exercised through a process-group SIGTERM and restart. the child's interrupted run was cancelled, its linked restart continuation completed, the parent received the final result, and its
task_statuscall returnedcompletedwithlatestTerminalStatus: completed. the complete flow and this capture were repeated on final headad0bf37ff4. no web, desktop-shell or mobile UI code changed.recovery benchmark
same blacksmith worker, same seeded workload and in-memory sqlite. target baseline
70e5a40c20versus candidateb8c3d46ead, milliseconds. each row is one measurement, not a statistical estimate. the baseline's candidate-count expectation was adjusted for its older held-queue selection; the timed matrix passed. candidate correctness and idempotence checks passed unchanged. disk/fsync costs are not measured.at 50,000 settled / 1,000 active threads, crash recovery decreased 18.8% and repeat recovery decreased 70.7%; candidate selection itself increased 5.5ms. these measurements include the intentional reduction in recovery candidates when queues are held.
the final SQL fix was also measured against the preceding PR head (
37f5e915dc), using the actual selection queries, matching sqlite indexes, one pending roster and identical returned rows. 2,000 / 5,000 / 10,000 runs took 568.59 / 3,732.02 / 14,631.07ms before and 0.69 / 1.44 / 2.71ms after. this isolates query scaling, not whole-server restart latency.two servers sharing one database remain out of scope; preventing that requires a single-instance lock.
implementation: opus 5.5 (high) via claude code and gpt-6-astra (high) via pi in t3 code. both independent gpt-6-astra (xhigh) reviewers approved final head
