🟠 High mcp/OrchestratorMcpService.ts:1551
Cancelling one delegated task also cancels unrelated queued work in the same child thread. queuedRuns includes every queued run, so additionalRunIds sends runs created by later sendToThread messages to queued-run.cancel without checking that they belong to this task's continuation cohort. Restrict the list to the task-owned cohort before constructing additionalRunIds.

ApprovabilityVerdict: Not approved Macroscope's review found this PR not approvable — This PR substantially changes delegated-task reconciliation, queue cancellation, provider recovery, and background-work settlement across core production orchestration paths. An unresolved high-severity finding indicates that cancellation may affect unrelated queued work, so the behavior and fix scope require human review. Not approved because:

506-506: 🚀 Performance & Scalability | 🔵 TrivialRecovery now scans every unpublished child at each startup.
The
subagent-resultsrecovery set now includes children that are still running or queued. At startup, each of these children runsfinalizeAppOwnedSubagentunder the parent lock. This matches the intent of the change. The cost of recovery now grows with the number of active children, not only children that have finished. Concurrency is bounded at 8, so this is acceptable. Also applies to: 3404-3407

Problem
Delegated children can finish several later turns while an older held follow-up remains queued. Their parent task and Lineage keep reporting Running, a dynamic MCP handoff item from a failed turn keeps the child waiting on background work, and
task_cancelrefuses because there is no interruptible provider run.The reported installed app was
0.0.45-preview.20261001.2556(source46de192f). A consistent read-only SQLite snapshot verified the same pattern in four Claude children: failed ordinal 1, unstarted held queued ordinal 2, later completed ordinals 13/9/12/4, idle provider, and an abandoned handoff item owned by ordinal 1.pendingRequestCount=0describes provider runtime requests, not the queued-message count. These four held requests are real and must survive recovery.Change
Reconcile unpublished delegated tasks from all outstanding child runs. Queued work projects as Pending/Queued; executing work remains Running; results publish only when the task settles. Startup and live run updates keep the task, graph node, and timeline item consistent. Provider recovery preserves app-owned children when their parent's provider session ends, and previously published results remain terminal.
Exclude dynamic tools owned by failed, interrupted, or cancelled runs from background waiting, while preserving their history. The shared helper covers web, desktop, and mobile; matching SQL projections cover server awareness and settlement. Native children, background commands, and real provider rosters retain their existing treatment.
Report queued and held-queued counts separately through thread inspection. Explicit task cancellation can cancel a queued-only task's selected queue cohort atomically, without promoting another member between cancellations or cancelling unrelated later work. The added wire fields are optional.
Scope and approval
This is a focused correction to the existing delegated-task lifecycle: durable status, pending-background derivation, and task controls must agree about the same outstanding work. The regression fixture establishes the observed failure and expected behavior without a new workflow or product setting. Related follow-up result-delivery work in #15004 addresses a different case; this fix preserves the existing published-result boundary.
