Problem

A command row shows the command, but not what it prints while it runs. Since V2 (#2829), it doesn't show the output after the command finishes either. Open issues: #6388 (Bash output never renders), #13950 (rows jump while a command runs).

Live output was never fully shipped, and each way it reached the timeline was later removed for performance:

• feat(streaming): implement command output streaming from runtime #1834 tried a subscribeCommandOutput RPC with a 24 KB buffer. It was closed unmerged.
• fix: cut websocket throughput in half by pruning activity payloads #4622 pruned raw tool output from activity payloads (websocket bytes, slow iOS startup on long threads).
• perf(server): cut idle CPU use and stop provider event leaks #8187 made every non-assistant content.delta an early return, which dropped Codex outputDelta (idle CPU, serial ingestion, retained provider events).
• fix(grok): bound cumulative tool output updates #7279 bounded ACP (Grok) output to an 8 KB tail with coalescing, because whole-buffer resends at ~10/s blocked ingestion (31.6 MB → 2.51 MB).
• perf(server): bound snapshot activity payload memory #9000, fix(server): bound slow-client event buffers #9715, fix(server): keep tool payloads out of completion queues #12345 and [Bug]: t3 repeatedly crashing on long thread, says it is network connectivity, but is actually do to unbounded query caused by Backend OOM crash when loading a thread with large tool-activity history #5351 bounded snapshot heap, slow-client buffers (1,000 items / 8 MiB), payloads retained in queues, and OOMs.
• d0fd9db ("perf(orchestration): bound v2 transport payloads", in feat(orchestrator): introduce new orchestrator #2829) made WireProjection drop command_execution.output of every size, including the final output (a 600-row snapshot went from 10.4 MB to 67 KB). This is the regression on main.
• fix: expanded tool calls show their output, empty ones don't expand #15505 (merged after this PR opened) brought back the final output: expanding a finished tool row fetches it once over getTurnItem, as raw text. A running command still shows nothing new while it runs, and colour codes and progress redraws show up as raw characters.

Fix

Output never becomes an orchestration event, and it never rides the thread stream.

• Adapters send raw output chunks to an in-memory command output hub before ingestion. The hub keeps a cleaned 64K-character tail per running command, with at most 64 running commands tracked, and drops the tail once the command settles.
• An expanded command row opens orchestration.subscribeCommandOutput for that row only. It sends the current tail, then appends at most every 100 ms. A viewer that falls more than 16 KB behind gets the whole tail instead, at most every 500 ms. The stream ends with the persisted final output.
• A finished row opened later gets its final output the same way, on demand.
• The terminal-output cleaner in @t3tools/shared/terminalOutput runs on both the server and clients. It models a small terminal. The last 64 lines form a window that \r, cursor movement, line erases and screen erases can rewrite, so single-line and multi-line progress (npm, curl, docker pull, pnpm, test reporters) keep only their latest frame. Older lines are fixed text. Colours and text attributes survive as canonical SGR codes that reset at each line end, so a cut never starts mid-colour. Other escapes and control bytes are dropped. Escapes split across chunks are held until complete.
• Web and mobile render colours from one light and one dark 16-colour palette, plus 256-colour and true-colour codes. Text on a coloured background switches to near-black or white when it would be unreadable (e.g. "PASS" badges). Past 2,000 styled spans the rest renders as plain text, so a pathological stream can't create tens of thousands of nodes.
• A resync (replace frame) carries the cursor position and current style, so a viewer that falls behind or reconnects applies later appends exactly as the server does.
• Capability-gated by commandOutputStreaming. Older servers never get the RPC.
• The thread snapshot and thread stream are unchanged. ThreadTransportPerformance.test.ts still pins the same sizes.

Codex	item/commandExecution/outputDelta. Codex itself doesn't stream what a command prints in its first moment (before its first yield). The recorded session has no delta for it, so that part appears when the command finishes.
Claude	The Agent SDK sends no Bash output until the result. Claude Code writes a foreground Bash call's output to <tmp>/claude-<uid>/<project>/<session>/tasks/<task_id>.output, naming the task in task_started (local_bash). The adapter tails that file every 250 ms. It's undocumented, so every step fails quietly to "final output only".
Cursor	Shell deltas go to the hub instead of the event store.
Grok, Antigravity (ACP), Pi	They already put bounded cumulative output in running item snapshots. The hub serves those snapshots to the open row.
OpenCode	Output arrives only at completion, so the final output shows on expand.

Per client

• Web and desktop: the expanded command row (V2ItemInspector, in fix: expanded tool calls show their output, empty ones don't expand #15505's tool-row layout) shows the live panel where fix: expanded tool calls show their output, empty ones don't expand #15505 shows its fetched output, and skips that fetch, so the output isn't loaded twice. • Local, remote and tunnel: this is an ordinary RPC stream on the existing websocket, with no transport-specific code. Only local was exercised by hand.

Taken over by @maria-rcks in a07cd90: 14 files, +162/-240, with no behavior from this description removed.

• The shared cleaner returns span styles that web and native style props accept as-is, so both panels drop their own converters.
• The panels trust their parent's capability check and take the theme they're given. The mobile exit code renders from one element.
• The hub reuses the contracts work-status helper, one bounded-map helper for both evictions, and Stream.takeUntil instead of an ended flag.
• Fixed along the way:
  • "Loading output…" now shows until the first frame. scan's seed used to hide it.
  • A line trimmed to its tail no longer starts on half an emoji.
  • The legacy Claude Bash JSON decoder only reads settled output with the full { stdout, stderr, interrupted } shape.

• Transcript-replay fixture command_output_streaming, recorded from real Codex and Claude sessions with the existing record-*-replay-fixture.ts scripts. It asserts that live output reaches the hub cleaned of escapes and progress redraws, that it agrees with the persisted final output, that the row shows stdout rather than a serialized tool result, and that no running command item ever persists output.
• CommandOutputHub.test.ts: identical text for every viewer, a flood sent as bounded tails at the slower rate, finished commands send only bounded final output, snapshot-only providers still stream, and interrupted commands keep their output.
• terminalOutput.test.ts: multi-line redraws, screen erases, terminal-style overwrites, colour codes surviving line ends and tail cuts, palette resolution, readable badges, the span cap, and a viewer that resyncs mid-redraw matching the source exactly.
• ClaudeTaskOutputTail.test.ts and the client-side commandOutput.test.ts.

Cleaner throughput (8 KB chunks): 17.8 MB of plain build log at 162 MB/s (was 36 MB/s), 4.4 MB of colour-heavy test output at 25 MB/s (was 42 MB/s with colours stripped).
