Modify view and controller to allow setting of a fixed reason when flagging

The horizontal ellipsis is a placeholder for the existing flags that have no reason (yet).

• Set a fixed maximum length in the server code e.g. don't let anyone go overboard and reject if they do.

I will fill out my reasons on my flags when this is complete and clear what is necessary... as promised I will clear all flags when this issue is complete and we can restart over since it will be better moderation with a reason.

This is next in moderation series marathon fixes.

Cleared all flags except for a few... those would be my Moderation keeps for now... filled in the reasons too manually to preserve the date.

Closing as fixed... open a new issue if there are any changes/enhancements wanted.

Just a misc. stat.... ~86% of the flags for Userscripts were upvoted e.g. most likely before we got the modal for flagging explaining what it is used for... however there are some recurring recent abusers... since this is a "fresh" restart on flag reasons... I've dismissed those for now... however abuse of the flagging system will most likely result in unfettered account removal as per the TOS.
