fix(signals): stabilize pr queue readiness

Summary

• Fix the PR panel Open PR queue row so score, evidence, and action use the same queue snapshot.
• Add queue evidence source metadata for cached, sampled, and authoritative likely-reviewable counts, with OpenAPI updated.
• Document the GitHub App install/setup path, health checks, settings preview, and Context-vs-Gate branch protection posture.
